1510937912 2017-11-17 Production Wells No Violations On the date of my inspection, I found a well maintained lease road to a wellhead equipped with a pumping unit, rods, 2.375 inch tubing, and 4.5 inch casing. There is an identification tag on the pumping unit. The tubing and casing are tied into a 2.375 inch production line. The tubing and casing valves appear to be in the open position. Pressure gauges on the tubing and casing show 0 psi but the well can be heard producing. The wellhead is protected by a steel tubing fence. There is a 1 inch steel line that splits into two 1 inch poly lines. One poly line runs through a drip and into a residential diaphragm meter while the other feeds an engine on the pumping unit. The production equipment is shared with permit 34-157-2-0920 includes two 210 barrel tanks, 2 vertical separators, 2 drips, and one 44.8 barrel steel brine tank within an earthen berm. There is proper identification on the tanks. Production records are current.
1529448363 2018-06-19 Production Wells No Violations Upon arrival on location, I witnessed the well was equipped with 4.5 inch diameter casing, 2.375 inch diameter tubing, rods, and a pumping unit operated with a natural gas motor. The valves were all shut, because of flowline leak. Production equipment included a steel 210 barrel tank, a steel 50 barrel tank, and a vertical separator within an earthen dike. The production report was up to date and proper identification was in place. Received a phone call from Bob Waylee from Enervest about a flowline leak. I met ODNR, Emergency Operations Coordinator Jason Blocker at the location to access the spill. The spill was about a 2-3 barrel release from the wells flowline. The flowline ran up a lowland/dry creek area. The spill consisted of an 80 foot diameter circle around the hole in flowline and then ran down dry creek for another 200 feet. Bob had a clean up crew on site and had started to clean the free oil with absorbent pads. They placed three different oil booms in the wet area of the dry creek. Clean up crew brought a mini excavator to scrape the top off the 80 foot circle around the flowline and to dig down to fix the hole in the flowline. The material they scrape off the top will be put in a dump truck and hauled to a landfill.

1928180123 2018-07-05 Production Wells No Violations Upon arrival on location, I witnessed oil booms had been staked down with metal rods. They had been changed out recently. Oil absorbent pads were placed in the dry creek above the oil booms. The owner was following instructions and the site looked good.
