0821544026 2015-04-29 Production Wells No Violations On the day of my inspection I found a 8 X 4 wellhead and a 4 X 1.5 inch wellhead, operated by a plunger lift system. The tank battery is made up of two 100 bbl tank, a horizontal separator, and a recording gauge. All in an earthen dike. The tank is not properly identified, and the production reporting is up to date. This well is being operated by an unregustered owner. The last year of reported production was 2012 by Endevor of Ohio. I belleive that Poorboy is operating the well. I will advise them to stop production until they become the owner of record.
1391305255 2015-04-22 Production Wells No Violations On the date of inspection I am following up on a Compliance Notice issued on 10-31-14 for a failure to legibly identify well. I found production equipment to include two 100bbl tanks and a horizontal separator within an earthen dike. The well is equipped with a gas plunger lift system, 1.5 inch tubing, 4.5 inch casing, and 8.625 inch casing. The tubing valve and one casing valve are in open position. 2012 was the last year that production was reported. There is no identification in place at the well or the tank battery. There is a 3 foot by 3 foot area within the dike that is contaminated with oil. I will be working with ODNR Inspector Randy Akins on determining the ownership of the well to have the issues addressed.. The current listed owner is no longer in business.
1414689329 2014-10-30 Production Wells Violations Noted On the day of my inspection I found a well with a 8 X 4 inch wellhead and a 4 X 1 1/2 wellhead, operated with a plunger lift system. The valves were in the open position. The tank battery was made up of two 100 bbl tanks, and a horizontal separator in an earthen dike. The tanks are not identified, I have placed a call to Tuco at 1:17pm and left a message to return my call. The tank battery is located approximately 100 feet away from a public highway.

The valves are in the open position. The production equipment is made up of two 100 bbl. tanks and a horizontal separator, and a recording gauge. The outlet valve on the separator is in the open position. The tanks are properly identified. Ownership has not been cleared up, but Poor Boy is producing the well.

Production equipment included two steel 100 barrel tanks, a horizonal separator, and chart recorder with an 2 by 1.250 inch orifice plate within an earthen dike. The production report was up to date and identification was present. The ownership of the well has not been cleared up from Tuco Partnership to Poor Boy Exploration. Poorboy Exploration has been operating the well.

2090781573 2016-03-22 Production Wells No Violations On the day of my inspection I found the well has been put back into production. The well has been idle do to a question of ownership. Poor Boy Exploration has alleged that they are the owner of the well, and are producing the well. I contacted David Bonar with Poor Boy Exploration and advised him that the well needs to be shut in. In the State of Ohio records Tuco Partnership is still the owner of record. I will contact bonding and surety to see if this matter has been resolved.
