The production equipment is made up of one 210 bbl. steel tank, a horizontal separator. The tank is properly identified, and the production reporting is up to date. The last production was in the year of 2013. The well needs plugged and abandon.
Northwood Energy needs to submit a written plugging schedule to Inspector Randy Akins at 2207 Reiser Ave. New Philadelphia, Ohio, 44663 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The owner/operator will be notified if the plugging schedule is approved or denied. |

They will tag the cement plug first thing, before they start trying to rip the 4.5 inch diameter casing. They tagged the cement plug at the depth of 6845 feet. They moved up the 4.5 inch diameter casing to the depth of 6000 feet, to attempt to rip the casing. The attempt failed, and they moved up the casing in 100 foot increments to the depth of 5000 feet. They then called Appalachian Well to come to location to perforate the casing at the clinton zone. Appalachian Well arrived and perforated the casing at the depth of 4986 to 4976, with 10 shots. The rig crew tripped 162 joints of 2.375 inch diameter tubing into the well. The tubing was set at the depth of 4986 feet. The cement crew established circulation on the well. Then pumped a 200 foot (60 sacks), Class A cement plug from 4986 feet to 4786 feet. They displaced cement with 18.5 barrels of fresh water to the depth of 4786 feet. Then the rig crew started to trip the tubing out of the well. |

On the day of my inspection, I witnessed the well was equipped with 8.625 inch diameter casing, 4.5 inch diameter casing, 2.375 inch diameter tubing, rods, and a pumping unit ran with an electric motor. The valves are in the closed position, the well was idle at the time of the inspection. The production equipment included a steel 210 barrel tank and a horizonal separator. The production report was up to date but showed no production since 2014. I contacted Bill Arnholt with Northwood Energy, he stated they are considering running a packer in the well to fix the 4.5 inch diameter casing. I told him I needed a written report for the procedure and a completion date for the project. |

The attempt failed, they called Appalachian Well to come and shoot the 4.5 inch diameter casing off at the depth of 3700 feet. The rig crew pulled 5 joints out of the well. This put them at a depth of 3480 feet. The cement crew established circulation on the well. Then, they pumped a 300 foot ( 86 sk), Class A cement plug. They displaced the plug with 51 barrels of fresh water. The rig crew started pulling another 51 joints of 4.5 inch diameter casing out of the well. This put the 4.5 inch diameter casing at the depth of 1236 feet. The cement crew established circulation on the well. Then, they pumped a 200 foot (60 sk), Class A cement plug. They displaced the plug with 16 barrels of fresh water. The rig crew started pulling another 24 joints of 4.5 inch diameter casing out of the well. This put the 4.5 inch diameter casing at the depth of 180 feet. The cement crew established circulation on the well. Then, they pumped a 180 foot ( 55 sk), Class A cement plug. They pumped cement back to surface. The rig crew started pulling the last 4 joints of 4.5 inch diameter casing out of the well. They topped the 8.625 inch diameter casing off with the remaining cement in truck. After a half hour, the surface, cement plug had dropped several inches below the top of the 8.625 inch diameter casing. |
