0604284260 2016-02-22 Drill / Deepen / Reopen No Violations On the day of my inspection I found, a sign board at the access road entry featuring the well names, permit number and emergency contact information. The access road has a padlocked gate near entrance. The access road is in good repair. I found the production pad to be enclosed entirely with a chain link fence with barbed wire at the top, and with a padlocked gate. The pad has a crushed limestone surface, and is in good repair. The well is constructed with a wellhead tree of 4 inch valves. There are pipe gates protecting the well from vehicles. The cellar is full of water, and has grating covering is to prevent someone from falling in. There is no production equipment on location. I am not able to determine if the well has been frac.

Chesapeake plans to move a top hole rig in the within a month, however, the permit for this well expired in April, 2014.

1543331214 2018-11-27 Temporary Inactive Well Status No Violations Upon my arrival on location, I witnessed the access road had been maintained. The location had not been disturbed. The well was in the idle position and no production equipment was on the location. No environmental issue was visible at the time of the inspection. The location was surrounded by a chain link fence with a locked gate. The earthen dike area inside and outside of the fence, had vegetation growing on it. The monitoring gauges had been installed, but the valves were closed and locked. Therefore, the pressure gauges were reading zero. I will contact Chesapeake to unlock the monitoring valves. So, we can monitor the annular pressures. Chesapeake has applied for a Temporary Inactive Permit. The permit is presently being reviewed.
1572850713 2012-05-03 Surface Facility Construction No Violations I was called out to location by Ashley. Crew thought they dug into an old drill pit while building a new location. I talked with landowner, Glen Mizer, and he advised what crew hit was an old dump site from the 1950s and not a drill pit.

1853645383 2019-05-30 Temporary Inactive Well Status No Violations This inspection is for a Temporary Inactive Well Status. Upon my arrival on location, accompanied by Southeast Supervisor Brent Bear and Gus Jerome from Encino, I witnessed the access road had been maintained. The location had not been disturbed and the sump valves were in the open position. The well was shut in and no production equipment was on the location. The monitoring gauges had been installed and the pressure gauges were showing no pressure on the annulus. No environmental issue was visible at the time of the inspection. The well cellar was full of water and we advised Encino to have the cellar pumped out. The location was surrounded by a chain link fence with locked gates. The earthen dike area inside and outside of the fence, had vegetation growing on it. We witnessed Gus Jerome completing the Inactive Status check list and signing it.
