At the time of the inspection, two gentlemen were observed at the well site shoveling dirt over areas impacted by contamination and pollution. He confirmed this deficiency and further stated that he does not have a lot of money and would like to abandon the well by removing the motor and just let it sit. Explained to Mr. Hartman the requirements for properly abandoning the well and that he would need to register, apply for plugging permit, locate a service rig. The plugging activity would be witnessed by the Division. The second option that we discussed was the possibility of selling the well to another company. |

A meeting was setup with her for 7/20/2009 to discuss registering with the DMRM as a well owner. Mrs. Booth was informed of the fluid and oil inside the dike. Absorbent pads could be used to retrieve oil or pumped to remove brine. She was informed that the 100 bbl tank further described as the bigger tank on top of hill contains a hole in bottom. Mrs. Booth asked what could be done. She was told that tank could be flipped, but would depend on topside condition. She states that they could look for a new tank. Also mentioned contaminated areas around well site had been buried, which may potentially resurface. |

Mrs. Booth states that Mr. Ron Huffines with Ohio Oil Gathering has pumped the tank, but would need to pump mor along with oil inside the dike. Further stating that old oil was hard to pump, but removed 3/4 of oil inside tank. Mrs. Booth states that the 100 bbl tank would be removed and a 50 bbl tank installed. Spoke with Mr. Riggs about earthen dike for tank battery and well head locations. Mr. Stonerock instructed both parties that the soil was to be removed from inside dike and bioremediation could be performed. Bioremediation could also be performed at the well location. |

After that, I noticed that one of the check valves was leaking gas, there were bare spots on the ground, the drip tank had residual oil around the cleanout valve, and the valve on the oil tank was still leaking. I spoke with an employee of the Zaleski Tavern, and she stated that Mr. Ray Hartman had told her that he didnt own that well. She said a man named Lemay owned it. I then explained to her that as a result of my courthouse search, the property went to Mr. Lemay but the well was still in Mr. Hartmans name. I gave her my card and asked her to have Mr. Hartman call me. |

I called Benny back and told him I compared my pictures with prior pictures and did not believe the spill has grown enough to warrant any additional costs. Benny agreed and is scheduled to begin the work on 4-27-2017. New photographs were taken during my inspection. |

An accumulation of old and fresh oil was observed around the well to include pumping unit. 2 x 4 has been placed near well head location, due to depth of fluid containing oil. Bubbling observed at this location. Facing north, bare area(s), old and fresh oil travel from 100 bbl tank, leaking rectangular tank and well towards the east and to the stormwater drain. At the tank location, on top of hill, fluid and oil were observed inside the dike and over the hillside. Note: Stormwater drain bends around property and drains into Raccoon Creek approx 300 away. The well is located approx 50 away from a section with runoff observed in south direction. Overall impact approx 75-100 x 5-50. Records indicate historical owner therefore, nearby residents were interviewed to inquire about well ownership. It was found that this well is owned by Mr. Ray Hartman, owner of the Zaleski Tavern, PO Box 206, Zaleski, Ohio, (740) 596-2629. Mr. Hartman was instructed to fix problem and haul contaminated soil taken to an EPA landfill. |

An oil sheen was not observed in the stormwater drain or at discharge side to Racoon Creek. Mr. Brent Bear and Mr. Mike Sherron located the well owner, Mr. Ray Hartman. Mr. Hartman was informed by Mr. Sherron that the contamination and pollution from his well site potentially threatens waters of the State. He was verbally instructed to cleanup the well site within 60 days and issued a Field Notice of Violation. Mr. Hartman was also verbally informed that his 100 bbl tank contains a leak at bottom. |

According to courthouse records, Ray is the owner of this well. The well has caused pollution and contamination. The oil spill is 57 feet long and approximately 5 feet wide. The 1 plastic line that is hooked to the casing is causing the leak. I called Annette at 10:05 AM and asked if she would be able to meet us. Annette agreed to meet us and arrived at 10:12 AM. We discussed the fact the well has caused pollution and contamination. Annette stated her father is on a limited income and did not have the money to perform the clean up. Annette also stated she did not have the money to clean up the pollution and contamination. We informed Annette the State of Ohio ODNR Division of Oil and Gas would hire a contractor to perform the clean up and she would be responsible for paying the bill. Annette was also told the State could place a lien on the property. Annette stated she would sign over the property to the State of Ohio as she had no ability to pay for the clean up. |
