Walked approximately 1500 of creek downstream and observed no crude oil. The total length of impact below hill was approximately 170 and width ranging from 15-50. Two sources for the release were found to be 1/4 valve with gauge broke off on the injection side of well. Lastly, a 2 black platic line from storage tank location to injection side may have leaked. Approximately 5 feet behind well next to line oil was observed. Spoke with Paul Grims employee, Mr. Paul Thompson whom states that this incident occurred last month. He states that the line broke and someone broke off gauge. Mr. Thompson was instructed to remove all contaminated material. Due to the terrain over the hillside, to include several trees and slope, potential erosion problems may occur from the operation of heavy equipment. Therefore, Bioremediation was suggested for this area. The estimated amount of oil released is unknown at this time, as rainfall events have occurred over the past month. Below hillside, the area consisted of mostly bare area with a few oil stains on ground surface. Unable to test injection side at this time. No pressure observed on the annulus. |

At location, salt crystals were observed on concrete. Another section of PVC pipe was observed in pit, but not connected to discharge pipe. The pit was found to contain 3/4 of oil on surface and measured 41 (length) x 16 and 22 (widths). High Range Quantabs utilized at discharge point exceeded 6545 ppm(mg/L) Cl-. A 4 oz container was utilized to measure discharge rate. The container was filled in 10 seconds. The storage tank for the ERP consists of a 200 bbl tank placed on concrete slab inside Mr. Paul Grims shop. Currently, no containment exists for the tank. At this location, the pump was found to be leaking continuously. The fluid from this leak then enters two floor drains that tie into the PVC pipe outside shop. Report continued on next page. |
