001269i831 2002-03-04 Production Wells Violations Noted An inspection of the well revealed that the well has no ID and there is no ID at the tank battery, also dike around tanks do not meet SPCC requirements. The dike has 3 - 100 bbl tanks and 1 - 210 bbl tank , they are not hooked up together, size of dike is approx. 25ft. x 30ft. x 1ft., dike capable of holding approx. 11 bbls of fluid. The dike needs to hold at least 230 bbls. Need full ID at the well and tank battery. Electric line needs hung from well to meter.

Erosion observed in several areas along the lease road. This issue will be addressed during the final restoration. Also noted was that the County was repairing Harvey Chapel Road (cracks in asphalt, ruts alongside road). Contacted Mr. Doug Kitchen regarding the plugging of this well. Mr. Kitchen stated that Mr. Grim told him to deal with the plugging and that he was not going to place cement in this well. Mr. Kitchen stated that he would contact Kilbarger to complete the plugging. The landowner was informed of the inspection results.

Chieftain Energy has been informed of this issue. Plastic and steel flowline remain along lease road entrance, tank battery, field, well site to include wooded area. A section of the lease road is now being used as a driveway and will require a waiver. The section of lease road past this location lacks vegetation. A 50 section of the lease road near the well site will also need to be graded. The well site will require grading, due to ruts and excessive soil pushed up against trees. Vegetation will need to be provided. The tanks have not been removed and location graded.

The contaminant consisted of vegetable oil and chunks of grease. Paul Grims employees were verbally instructed to acquire freshwater. Doug Kitchen was informed by telephone of this incident. He states that our fluid level must have gotten down to the bottom of his tank. He was informed that this truck is to only be used for UIC-Brine and that freshwater would be needed.
