0078536940 2009-05-05 Production Wells No Violations A follow-up inspection was conducted to check on the status of oil spill cleanup. It was noted that the cleanup process is on-going, as emulsified oil was observed to have accumulated, in a roadside culvert that contains a siphoning dam. Upstream from this location, in the intermittent stream, a small pocket of oil accumulated around a sharp bend. Oily leaves (old oil) still exist along the bank of this stream. Ohio EPA had previously instructed the well owner to compost the contaminated material and not to burn. The well owner has been working towards achieving compliance.
0107553828 2009-06-29 Production Wells No Violations An inspection revealed that the siphoning dam has now been built back up. No observed contamination and pollution in culvert. Upstream, the ravine was dry at time of the inspection.
0155671199 2009-03-13 Production Wells No Violations A complaint was received alleging that an oil spill has occurred on Creola Hue Road. Arrived on-site and spoke with Mr. Mike Sherron, Ohio EPA and Mr. Barney Cook, U.S.T. Environmental Contractor. Mr. Sherron states that the Division of Wildlife has been contacted. An inspection of two roadside drainage culverts and a tributary revealed the presence of oil. Note: darkness hinders observation of total impact from this spill. U.S.T. Environmental states that the cleanup will begin tomorrow morning. Both Mr. Sherron and Mr. Cook have installed containment. Mr. Sherron spoke with the well owner, Mr. Timothy Jubach. According to Mr. Sherron, Mr. Jubach had stated that Permit # 737 has not been pumping, due to an electrical problem. These two wells pump into a common tank.
0275271938 2009-04-10 Production Wells No Violations An inspection was conducted with Mr. Mike Sherron, Ohio EPA, to check on the progress of oil spill cleanup. It was noted that oil still exists in the stream. Walked the ravine and pasture field and unable to visibly observe the flowline that had leaked.
0534845148 2009-03-19 Production Wells No Violations An inspection was conducted with Mr. Mike Sherron, Ohio EPA, to check on the progress of oil spill cleanup. The cleanup is still on-going, but significant progress has been made. Small pockets of oil, in the stream, were observed around bends. An attempt was again made to trace the flowline from well location to the tank battery. Note: The well owner does not have a record/map of flowline. Walked the ravine and pasture field and unable to visibly observe the flowline.

1245395119 2009-06-09 Production Wells No Violations A follow-up inspection was conducted with Mr. Robert Stonerock, South Region Oil & Gas District Supervisor. During the inspection, it was noted that the cleanup appears to have been completed. The siphoning dam has washed out due to rainfall events. No oil was observed in roadside culvert. The well owner, Mr. Timoty Jubach, will be informed to build up the back wall of earthen dike. Furthermore, if spill occurs again the earthen dike will need to be lined. As previously noted, the flowline was never located leaving two sources buried underground line within the ravine or tank battery location. It has also been noted that after rainfall events water does not stay inside dike per Mr. Jubach.
1263597425 2009-03-25 Production Wells No Violations A follow-up inspection revealed that progress has been made with the oil spill cleanup. It was noted that a small amount of oil still exists around bends, in the tributary, and shows signs of breaking down. Ohio EPA had supervised and coordinated containment measures. The well owner states that the flowline will be abandoned and a tank will be placed closer to the well location.
1911121022 2009-03-16 Production Wells No Violations An inspection of the oil spill cleanup was conducted in coordination with the Ohio EPA. Ohio EPA representative, Mr. Mike Sherron, states that 300 gallons of product has been retrieved from the intermittent stream. The cleanup contractor, UST Environmental, is skimming the surface of the water at the roadside cuvert location with their pump truck. The source of the spill is believed to have been a steel flowline from this well. The flowline is routed within the ravine location, resulting in the migration of oil underground and seepage into the intermittent stream.
1984575502 2009-03-14 Production Wells Violations Noted Arrived on-site and found that U.S.T. Environmental Contractors have begun oil spill cleanup. An inspection of the tributary from a roadside culvert to a location approximately 1/4 mile south where oil stops was conducted. The overall impact of oil, in this area, was intermittent. Oil was found in sections and more notable in pockets around sharp bends. A few areas also showed signs of emulsification. An inspection of the ravine was conducted. This section starts from the opposite roadside culvert to a northeasterly direction towards the well and tank. Oil was observed, in a intermittent stream, to a location ending approximately 300 feet downstream from a pond and greater than 1000 feet from the well. An attempt to locate the flowline near this location was not successful, due to the topography and burial of pipeline. Note: Tank is located at a higher elevation and downstream approximately 100 feet. The total length of impact is approximately 1/4 mile. The pond was inspected and showed no signs of visual contamination or observed flowline nearby. The tank storage consists of (1) 210 bbl for two wells. The tank was inspected, but showed no signs of leakage and appears to be in good condition. At this time, it is believed that the source of the leak could be from a broken steel flowline located underground. The well owner will be contacted regarding the line location and well operation.
