According to Mr. Sherron, Mr. Jubach had stated that Permit # 737 has not been pumping, due to an electrical problem. These two wells pump into a common tank. |

The well owner, Mr. Timoty Jubach, will be informed to build up the back wall of earthen dike. Furthermore, if spill occurs again the earthen dike will need to be lined. As previously noted, the flowline was never located leaving two sources buried underground line within the ravine or tank battery location. It has also been noted that after rainfall events water does not stay inside dike per Mr. Jubach. |

An inspection of the tributary from a roadside culvert to a location approximately 1/4 mile south where oil stops was conducted. The overall impact of oil, in this area, was intermittent. Oil was found in sections and more notable in pockets around sharp bends. A few areas also showed signs of emulsification. An inspection of the ravine was conducted. This section starts from the opposite roadside culvert to a northeasterly direction towards the well and tank. Oil was observed, in a intermittent stream, to a location ending approximately 300 feet downstream from a pond and greater than 1000 feet from the well. An attempt to locate the flowline near this location was not successful, due to the topography and burial of pipeline. Note: Tank is located at a higher elevation and downstream approximately 100 feet. The total length of impact is approximately 1/4 mile. The pond was inspected and showed no signs of visual contamination or observed flowline nearby. The tank storage consists of (1) 210 bbl for two wells. The tank was inspected, but showed no signs of leakage and appears to be in good condition. At this time, it is believed that the source of the leak could be from a broken steel flowline located underground. The well owner will be contacted regarding the line location and well operation. |
