0311244521 2017-10-02 Production Wells No Violations On day of this Follow-up inspection to Compliance Notice # 1493286220. My inspection today finds, vegetation growing back nicely on the downslope. The containment impound below downslope was still holding water. I contacted Inspector Matt Fulk to bring me some Quantabs to test the water impoundment on the downslope. The containment impoundment below had frogs, salamanders and other living organisms in it. We tested with Quantabs and found the water to contain approximately 756 ppm. I will continue to monitor the chlorides in the impoundment.

Currently waiting on the Company to mobilize equipment to remove the tanks and remove the contaminated soil.
1205869332 2018-06-28 Production Wells No Violations Compliance notice # 1493286220 follow up inspection. The water in the down slope impoundment was tested with high range Quantab, the results were 3.4 on the strip which = 541 ppm Cl. High range Quantab was also used to test the water seeping out of the sandstone ledge. The results were 2.8 on the strip = 402 ppm Cl. Water at the small impoundment surrounding the transfer pump was tested with High range Quantab, the results were 2.8 on the strip which = 402 ppm Cl. Vegetative growth has been established on the slope between the 2-210 barrel tanks and the impoundment. The site has been saturated from recent rain events.

1471992045 2018-08-29 Production Wells No Violations The purpose of my inspection was to determine if the compliance notice written for pollution and contamination associated with this brine transfer station could be written off. I sampled the containment pond downhill of the tank battery using high range quantabs. I did not get a reading off chlorides using the quantabs. The slope starting from the tank battery going down to the containment pond has thick vegetative growth. This satisfies the compliance notice.
1493286220 2017-04-26 Production Wells Violations Noted The brine transfer facility for the Beckley 3416320883 and the associated common tank battery for the Ploski lease has released oil and brine. The contaminated area is 45 feet by 22 feet. The area is void of vegetative cover . Down slope from the common tank battery and the poly tank, brine and oil were observed flowing out of the ground, High Range Quantabs of the water revealed 4429 ppm Cl. The Base of the slope has been impounded and is currently holding water. The impounded water was tested with High Range Quantabs at 2284 ppm Cl. Mike McPeak was called and responded to the location to discuss the contamination and remediation. The poly tank and one steel 210 barrel tank will need to be removed. The contaminated soil must be removed and taken to an EPA approved landfill. Mike was instructed to shut off production to the facility until remediation is completed. Mike was also told to place a common tank identification on the tank battery. Trisha Cartmell ODNR DOGRM on call emergency responder and The Vinton County Oil and Gas Inspector were notified of the release.
1494271771 2017-05-08 Production Wells No Violations Bancequity on site removing brine contaminated soil. The soil was tested using quantabs. The contaminated soil was taken to the Athens Hocking landfill for proper disposal.
1494592705 2017-05-08 Production Wells No Violations Compliance notice # 1493286220 follow up inspection. The 210 barrel steel tank and the poly tank have been removed from the location. Bancequity crew is excavating contaminated soil and hauling it to the land fill in Logan Ohio. The water in the down slope impoundment was tested with high range Quantabs the results were 8.4 on the strip which = 5521 ppm Cl. High range Quantabs were also used to test the water and small amount of oil seeping out of the sandstone ledge. The results were 6.2 on the strip = 1925 ppm Cl. Field tests of the soil in the excavated areas revealed less than 200 ppm Cl. The Bancequity crew is going to place top soil, seed and mulch on the excavated areas to establish vegetative growth. The seep at the sandstone ledge has been left open to allow observation. The fluid in the impoundment is to be removed and stored in a tank until injected in an approved class 2 injection well.
1494616599 2017-05-12 Production Wells No Violations Compliance notice # 1493286220 follow up inspection. A common tank battery identification is posted on site. A used steel 210 barrel tank has been installed. The brine transfer pump drain pan has been plumbed to a plastic barrel with a sump pump . The sump pump is piped to the 210 barrel tank. Grass seed, lime, and mulch have been applied to the excavated area between the tanks and the down slope impoundment. I talked to Jess Hess about the need to pump out the impoundment water and place the water into a tank for settling then injection at an approved class 2 injection well. No oil was observed at the sandstone ledge.
1506963140 2017-10-02 Production Wells No Violations Inspector Jimmy Wiles contacted me and asked if I would bring him Quantabs to test the downslope area that was still holding water. Upon arriving at the location the vegetation on the downslope area was growing back nicely. The containment impoundment below had frogs, salamanders and other living organisms in it. We tested with Quantabs and found the water to contain approximately 756 ppm. Inspector Jimmy Wiles will continue to monitor the chlorides in the impoundment.
1517494856 2018-02-01 Production Wells No Violations On day of this inspection to Compliance Notice # 1493286220. I witnessed vegetation growing back nicely on the downslope. The containment impound below downslope was still holding water. The containment impoundment below is still holding water during my quantab testing. The testing of water using quantabs found that the chlorides levels have dropped approximately 289 ppm since last inspection. I will continue to monitor the chlorides in the impoundment to insure that chlorides are still lowering.
1530897059 2018-07-03 Production Wells No Violations Compliance Notice # 1493286220 follow up inspection. Meet on site with ODNR/ DOGRM ER Lane. The site was inspected and the soil was tested using ER Lanes state issued conductivity probe. The site was passed by ER Lane. I talked to Mike McPeak 7-3-18 at 10:09 am about the need to use a plastic liner under his brine transfer pump. The transfer pump is leaking a small amount of brine into an unlined containment area with a sump pump to pump fluids into the 210 steel tank. High Range Quantabs were used at the transfer pump site. Results were: 4.4 on strip = 821 ppm Cl.
