0423526347 2018-12-10 Production Wells No Violations My inspection revealed the well consists of a wellhead with tubing. The well was connected to a production line. The valve on the production line was in the closed position. The last reported production was in 1991. The well lacked a legible identification. The owner of the well shall place a legible identification at the wellhead. The well is currently incapable of production. The well owner, ADAMS OIL & GAS LLC shall submit a written plugging schedule to Kip Thompson at 5880 Memory Road, Zanesville Ohio 43701 within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

Mr. Adams has stated to me he cannot work on this well to correct the leak on the advice from his legal council. Mr. Adams erected a fence around the wellhead. I did not detect any natural gas using my gas detector. This day was windy and the wind was dissipating the gas.

Mr. Adams returned my call at 11:01 AM. I asked Mr. Adams why he has not corrected the natural gas leak at the wellhead. Mr. Adams stated his legal counsel is currently attempting to have Unified Investments return the assignment for this well. Mr. Adams stated this wells assignment is recorded in the Vinton County courthouse and is assigned to United Investments. Mr. Adams stated once he receives the assignment from United Investments he will repair the natural gas leak.

1408849879 2019-02-25 Production Wells No Violations My inspection revealed this well continues to leak natural gas. The owner of the well, Mr. Doug Adams has installed a fence around the wellhead. Mr. Adams has told me on the advice of his legal counsel, that he should not preform any additional work on this well. Pictures were taken today and was sent to management.

At 9:16 AM, I left a voice mail message for well owner Mr. Doug Adams. My message was to return my phone call. The fence that surrounds the well is still in place. This fence was installed by Mr. Doug Adams. Mr. Adams returned my call at 10:23 AM. I informed Mr. Adams he shall repair the leaking gas well. Mr. Adams stated on the advice of his attorney he was not to work on the well.
1624882922 2019-02-15 Production Wells No Violations The owner of this well, Mr. Doug Adams arrived at 11:00 AM. Mr. Adams used a shovel to uncover the conductor casing and found the well was leaking gas between the surface casing and the conductor casing. I informed Mr. Adams the leak was his responsibility to repair. I contacted ODNR emergency response via email regarding my findings.
1788334175 2008-08-13 Production Wells No Violations An inspection revealed that the well is idle and not capable of production. The valves were observed closed and the flowline was found cut. Records indicate no production. No identification was observed. Mr. Doug Adams, Adams Oil & Gas, was contacted by telephone on 8/14/2008, and informed of the inspection results. Mr. Adams confirmed that this well is idle. Mr. Adams states that he is working out an agreement with Kilbarger that would allow him to sell gas from this well. He states that there are also negotiations with Columbia Gas Transmission.
1923233444 2019-02-14 Production Wells No Violations My inspection revealed this well is leaking natural gas. Upon arriving at location, I was approached by the neighbor who lives approximately 70 feet east of this well. He stated the well was leaking. I approached the well and could hear natural gas leaking from this well. The neighbor stated he was very concerned about his familys health and safety with the well leaking natural gas. I contacted ODNR emergency response divisions to inform them of this leaking gas well. I entered the neighbors complaint into the divisions complaint database. At 2:00 PM I received a call from Mr. Doug Adams who agreed to repair the gas leak on the morning of 2.15.19. I have informed the neighbor of my findings.
