The Quantab recorded 6.8 (2050 ppm Cl). The containment dike had 1 inch of water in the dike (from a recent rain event) just opposite of the bare area. The water in the containment dike was 1.2 on the high end Quantab. I called mike McPeak and we discussed the need to remediate the bare area and to keep the containment dike free of water.

Mr. Kitchens had coordinated this meeting with the landowner and Mr. Kitchens attorney, due to a restraining order that is in effect. The landowner failed to show after 1.5hrs to unlock the gate. As a result, another date will have to be set to resolve the violations. Mr. Kitchen was instructed to schedule another date immediately.

Containment dike was full of oil and water up to three inches from the top of the dike wall. The containment sump pump was in operation during the inspection and the fluid level did decrease approximately 1 inch during the inspection. The 100 barrel steel tank appears to have over flowed oil into the dike. I was unable to close the 1/4 valve on the injection tubing, so the company pressure gauge was re tightened. I was unable to open the 2 valve on the annulus. 20 psi was observed between the 2 valve and the 1/4 valve on the annulus. I called Mike McPeak at 1:51 pm and left a message outlining the need to continue to remove the fluids from the containment dike and that the 1/4 injection tubing valve needs replaced along with the 2 annulus valve. I called at 5:34pm and talked to Mike McPeak and we reviewed the dike clean up efforts and the need to replace the injection tubing valve and the annulus valve so that the pressures can be monitored.

1426191458 2015-03-12 Enhanced Recovery Project Violations Noted Follow up inspection of the surface facility revealed that the oil and water had not been removed from the containment dike. The containment dike sump pump was turned off. Two brine seeps and scald areas were observed down slope, South of the surface facility containment dike. 1st measured 6 feet by 6 feet, 2nd measured 21 feet by 3 feet. High Range Quantabs were used to determine the extent of the contaminated area. Both scald areas exceeded 6078 ppm Cl (mg/L). All Fluids in the containment dike must be removed and placed in tanks. The containment dike must be water tight. Repair or replace the containment dike so that it is water tight. Remove all contaminated soils and take them to an EPA approved land fill. Remediate the contaminated areas.
