Demostrate well is producing. Post IDs. Submit annual production. Add to bond. Failure to comply may result in bond forfeiture. 3/21 - RESOLVED - well pumping, good IDs, well added to Gas Ent.s bond.
0753328616 2012-09-25 Production Wells No Violations My inspection this day was to check brine kill location and see if vegetation was growing back, creek free of all chlorides, and wells back in production. I found kill area about 10 x 30 still had no vegetation but there were clumps of lime, from treating the soil, that were still visible. The creek tested fresh with no sign of brine. I will continue to monitor and not resolve violation until all vegetation has grown back. Wells still are not being produced until a solution can be found for the water.

1340306320 2012-06-19 Production Wells Violations Noted My inspection this day found brine water running out of a 1 hose and onto the ground as a means for disposal of brine. This find was due to a complaint by adjoining landowner, Steve Hendershot, who was cutting brush along the property line. The kill area is located on the south end of the property, owned by Jim Williams who is also the Operator of these Courtney wells in the E/2 of Section 21. The kill area is @ 25 x 75 and the brine coming out of the hose is very strong. The wells could be classified as Exempt Mississippian, as they meet all the requirements, but the brine is killing vegetation and too strong to be put on the ground. The brine has travelled thru the soil and is coming out in a creek @ 10-20 below. Sampling of the stream found contamination for 1.25 miles where another tributary joins in and the water clears up. There is not much oil but ground and rocks are orange/red from a high concentration of iron. Producer was notified and has shut wells down until further notice. My notice received @ 10 AM and wells were shut down @ 3 PM. As the stream is flowing, the water in the stream immediately started to clear up. The OEPA notified as well as Rocky King, supervisor. Operator is cooperating 100% and had started the clean-up that evening by covering contaminated area with lime and had also placed some in several spots in the creek. Samples were taken as well as photos. 9/21/12 - Returned money order/check made out to ODNR - Oil & Gas Division to Jim and instructed him to sign a new Consent Agreement without my name on it, and to get a Cashiers Check made out to State Treasurer, or whatever was required in Consent Agreement. Old check had been mailed to Zanesville office and just found - Jim is working on replacement check for $2500.
1343142324 2012-07-24 Production Wells No Violations My inspection this day of the Ferrell sample site found contaminated water has cleaned up and is now pure. I took water sample and will have it analyzed to confirm. Once results come back confirming this, violation can be resolved.
1925699792 2013-12-11 Production Wells No Violations My inspection this day with Andy Thomas to resolve issue of brine dumping as related to this lease. Ground was found to be growing vegetation, no evidence of further brine dumping. I believe this was a Chiefs Order that has been resolved. RESOLVED
2118873101 2012-06-27 Production Wells No Violations Jim Williams called to say he had run 1260 bbl of water down thru the creek last Thursday (6/21) night. Plans to do it again as we still have not received rain. Checked the Ferrell lease (Sample #6) and the creek still contained some brine but not as strong as last week. Also some orange staining from the iron.
