In addition, dead vegetation has been cleared from the NE edge of the wetland area to allow room for equipment to be brought in to begin excavation of contaminated soil. Photos were taken.
1070806666 2012-07-19 Production Wells No Violations My inspection this day found Chad Hall pumping wells. Found well fluids running from hose into road ditch. Sample tested with Quantabs and found to be ________ppm chlorides. While these wells meet all criteria for Exempt Miss., test results concern me. There is no noticable vegetation kill but told operator that his actions could be tested in court. I advised to be more descreet with his dispersing of fluids and that he could continue until a kill area developes. Also discussed his identification, pointing out that he was using wrong numbers and she he would get it corrected in the next week.
1388196321 2016-05-10 Production Wells No Violations Upon inspection, it was observed that grass is growing well on the previously affected area along the edge of the wetland. Quantab field test strips indicated a chloride level of 483 ppm in the water along the edge of the wetland and 237 ppm in the water along the bottom in the middle of the affected area of the wetland. Sweetflag rhizomes which were planted by Mr. Hall appear to be healthy and growing. Photos were taken. Tom Hall called at 11:42 am stating he has soil sampling results from the wetland substrate. A meeting has been scheduled for 12:00 pm on 5/11/16 to look at results and discuss further remediation.
1438738494 2016-04-22 Production Wells No Violations On location with DOGRM Emergency Operations Coordinator Nathan Lane to inspect wetland cleanup efforts by the operator. Mr. Hall failed to notify the division of cleanup activity, so the cleanup efforts were not witnessed. Upon inspection, it was observed that the deep ruts present around the wetland from equipment operation have been smoothed out and the area has been seeded and mulched. Mr. Hall pulled rhizomes from surrounding hydrophitic vegetation and planted them in random clumps in the brine affected area of the wetland. There is still excess topsoil in the water of the wetland which should be removed. Hach Quantab field samples of wetland water in the brine affected area read 997 ppm chlorides, which is an increase from the last inspection on 4/19/2016. Recommended cleanup guidelines were provided today by OEPA Wetland Ecologist Brian Gara. I will contact Mr. Hall and schedule a meeting at the location to discuss further cleanup requirements to meet OEPAs guidelines.
1460573600 2016-04-07 Production Wells No Violations At 10:30, Jake Glascock, Nathan Lane, and I met on the location with Scott Shane of OEPA, John Wellman of BCI, and Thomas Hall. We discussed violations, disposal options for contaminated soil, and time frames for remediation. Scott Shane indicated that OEPA will be issuing a compliance notice citing ORC. 6111 discharging into waters of the state, and will work with John Wellman to investigate further and determine if further action will be taken. Mr. Shane also stated OEPA would be deferring remediation oversight to DOGRM staff. Mr. Hall was advised to contact an OEPA approved landfill to discuss soil profiling requirements before attempting to dispose of the contaminated soil. I will be issuing a compliance notice citing ORC. 1509.22(A) and .22(C) with a 21 day time frame for compliance. Mr. Hall has verbally agreed to comply with remediation requirements.
1461845176 2016-04-26 Production Wells No Violations On location with SW Region Supervisor Dan Goins and Region Geologist Jake Glascock to meet with Tom Hall and discuss further requirements to reach compliance. Mr. Hall was given a copy of wetland remediation recommendations, which were supplied by OEPA Wetland Ecologist Brian Gara. Mr. Hall was urged to contact OEPA for any further guidelines to follow as far as remediation. Mr. Hall verbally agreed to comply with further remediation requirements.
1479418257 2016-04-06 Production Wells Violations Noted Performed an inspection of this well location after discovering a large area of dead vegetation in a wetland area along Sheets Run Rd. Upon inspection, it was observed that the well is currently in production and consists of a pumping unit standing over 6.2625 casing, 2.375 production tubing, and 1 tubing on a gas escape packer. The tank battery consists of 2 100 bbl production tanks, 2 small brine knockout tanks, and a containment dike. Both of the brine knockout tanks have 1.5 plastic disposal lines which run 75 to the south and discharge into a wetland. There is a vegetation kill area within the wetland which measures 50 long by 35 wide. The water in the wetland near the discharge point was field tested with Hach Quantab chloride titration strips. Field testing results indicated a chloride level of 4815 ppm chlorides. In addition, there is a significant amount of crude oil present in the wetland and on the soil surrounding the disposal lines. Brine and other oilfield waste may not be disposed of in this manner. Surface water samples were taken and submitted to the ODNR Environmental Lab for analysis. Photos were taken to support the findings of this inspection. The operator, Thomas E. Hall, will have 21 days from the issue date of this notice to have the following remedial tasks completed: 1.) Clean up all crude oil present in the wetland area; 2.) Excavate all contaminated soil from inside and outside the wetland area and dispose of it at an OEPA approved landfill; 3.) Eliminate all illegal disposal lines and arrange provisions to dispose of produced brine legally.

1775548357 2016-06-10 Production Wells No Violations Upon inspection, it was observed that cleanup efforts for the well location are complete. Planted vegetation in the wetland area is living despite low water levels. The operator has been instructed to monitor the wetland for progress over the next year and will sample the substrate in the spring to check for the presence of residual chlorides. All disposal tickets and sample documentation have been provided to DOGRM by the operator. Brine is no longer being disposed of illegally. This satisfies the requirements of the compliance notice dated 4/7/2016.
1879489625 2016-04-26 Administrative Inspection No Violations Received a call from Tom Hall at 6:27 pm regarding collection of soil samples from the affected wetland substrate. It was recommended to Mr. Hall that he contact an environmental laboratory of his choice and request sampling protocol to follow. It was explained to Mr. Hall that results of the samling event would dictate what further remediation requirements would entail.

Transplanted sweetflag rhizomes are beginning to take root in the affected area of the wetland. Water from the bottom of the wetland was field tested with Hach Low Range Quantabs and registered chloride levels ranging from 95-275 ppm, which is significantly lower than the last inspection. As per OEPA recommendation, Tom Hall has been instructed to grab soil samples from the affected area to determine if further excavation will be required. Photos were taken.

1945812197 2016-04-19 Production Wells No Violations On location with DOGRM Emergency Response Coordinator Nathan Lane to meet with Tom Hall, OEPA Investigator Scott Shane, and OEPA Wetland Specialist Brian Garra to discuss further cleanup of the brine/oil contaminated wetland. Upon arrival, it was observed that excavation efforts had taken place over the weekend and new soil had been brought in to replace the old. Mr. Hall provided soil profile results and a landfill disposal ticket. There are several deep ruts leading into the wetland from the large tractor used to scoop the soil out. Mr. Hall was instructed to bring equipment back in to smoot out the ruts, put the bank of the wetland back to contour, and seed/mulch the soil. Mr. Garra instructed Mr. Hall to pull rhizomes of the existing wetland vegetation and transplant it into the affected area to ensure the native vegetation takes back over. Mr. Hall was also instructed to remove all old unused lines from the wetland and surrounding area. Water/soil mixture from the affected area of the wetland was field tested with Hach Quantabs and showed results ranging from 187 to 421 ppm chlorides, a significant reduction since the last field test performed. New photos were taken of the location.

2057590844 2017-05-05 Production Wells No Violations The inspection this day revealed that this well is equipped with a pumping unit, a 4 1/2 inch diameter wellhead, and 1 1/2 inch diameter production tubing. The valves on the wellhead are open at the time of inspection. The tank battery for this well is equipped with a 100 barrel steel tank with proper identification on it. There are no issues to report and this well is in production at the time of inspection.
