0037373400 | 2004-05-17 | Production Wells | Violations Noted | Examined oil spill clean-up area - some oil still behind booms, have got main oil cleaned up at beaver dam, fair rainbow still coming down creek immediately below the tank but still need good rain to finish moving the oil. No oil at boom where Rainbow meets Muskingum so will pull that boom today and return to Artex. Will just continue to monitor daily. |
0038948680 | 2004-07-01 | Administrative Inspection | No Violations | Talked to Larry Lang - getting new bottom put in tank - should be back next week - location all ready for it - will call me when completed. |
0179904919 | 2004-05-12 | Production Wells | No Violations | Inspection this day found well with jack producing, tank has hole in bottom and has resulted in oil spill, 5-7 miles down Rainbow Creek. Amount lost was estimated at 50 bbl. By landowner/operator but Dennis Deavers of EPA estimates only 10 bbl from looking inside tank. Landowners said hadnt sold oil in year or 2 so I would think more than 10 in the tank. Good rainbow all the way down creek, with oil laying trapped in isolated pools --up. He is 86 year old man that just got out of hospital from having a stroke. B-D Oil Gathering has agreed to conduct clean-up. 5/18/04 - Cleanup indicates oil loss @ 20-30 bbl - much less than first predicted. 7/28/2004- Tank still not in place - well not producing |
0401208758 | 2004-05-27 | Production Wells | No Violations | Inspection this day found creek running clean, even up near tank, tank has not been removed and replaced but water now good, one boom floating downstream. Must have gotten good rains - way up! |
0832455437 | 2004-06-11 | Production Wells | No Violations | Inspection found old tank has been removed, area below the bad tank cleaned up and constant rains have cleaned the area - talked with owner and new tank should be in next week or two - trying to find one - will give me a call when complete. Stream looked good. |
1244845222 | 2004-08-19 | Production Wells | No Violations | Re-worked tank set today, good IDs, good dike, redid lines to assure no leaks. NOV resolved |
1825055981 | 2004-07-28 | Production Wells | No Violations | Still no change - reworked tank still not set - waiting on L. Lang. |
