0051225949 2016-06-28 Final Restoration No Violations The purpose of my inspection today is a status check on a final restoration. During my inspection I witnessed one 100 bbl. tank remains within an earthen dike. Tank is empty. Location has not met 1509.072 B requirements. Dike and the production equipment need to be removed before the final restoration can be passed.
1430313156 2015-04-28 Plug / Plug Back No Violations On location to witness plugging operations for this well. Upon arrival, B&E Resources had run 1920 of 1.5 tubing in the hole to spot the bottom plug. Formation Cementing pumped a 400 bottom plug using 26 sks of 2% gel, 1% CaCl cement with LCM and 3.8 bbls of freshwater displacement. Tubing was then pulled OOH and Jet Wireline Services arrived to run a bond log and shoot the 4.5 casing. Free pipe was found at 820 and the casing was shot off, but was found to be hung in the hole. Formation Cementing then pumped on the 4.5 casing in attempt to clear the annulus, but was unsuccessful. Jet Wireline then ran another bond log from 700-surface, found free pipe at 460 and shot it off. The shot was successful, but the casing was left in the hole to allow 1.5 tubing to be run to the initial shot location at 820 to spot a plug. The well will be plugged out tomorrow morning.

1675218315 2015-04-29 Plug / Plug Back No Violations Upon arrival, B&E Resources had run 860 of 1.5 tubing into the hole to set a 150 plug through the cut 4.5 casing. Gary Lang of B&E Resources stated that his crew dug down 4 around the 8.625 casing and found that the 10.75 surface casing and 13.375 conductor had been pulled from the well. The decision was made to leave the 8.625 casing in the well to provide protection from surface subsidence as well as protect shallow freshwater. Formation Cementing pumped 20 sks of 2% gel cement and 1.7 bbls of freshwater displacement. Tubing was then pulled OOH and the remaining 460 of 4.5 casing were also pulled OOH. Jet Wireline Services then perforated the 8.625 casing with 4 holes at 345 and 4 holes at 100 to allow cement into the uncemented annulus. Tubing was then run to 363 and a 200 plug was pumped using 80 sks of 2% gel cement. Tubing was then pulled to 160 and cement was circulated to surface with another 66 sks of 2% gel cement. The cement slurry volume was equivalent to filling 363 of 10 hole, indicating that the perforations took an adequate amount of cement. Cement stayed at the surface. Good circulation was achieved on all plugs. I was accompanied by Inspector Matt Fulk for the duration of this job.
