0830108825 2016-06-21 Final Restoration No Violations The purpose of my inspection today is a status check on a final restoration. During my inspection I witnessed no evidence of an oil and gas well and the site has been restored. Location has met 1509.072 B requirements.
1520870541 2015-09-25 Plug / Plug Back No Violations On location to check final restoration status for this well. Upon inspection, it was observed that the lease road is rilled out, eroded, and void of vegetation in several sections. The former well location is barren in areas and the pit location, while closed and graded, is still very wet and soft. I tested water ponded up in the former pit location with a Low-Range HACH Quantab strip and it tested less than 250 ppm chlorides, indicating it is likely ponded up rainwater. I will contact Mr. Chuck Ungurean of Lakeside Inc. and tell him to address final restoration issues on this location or to provide a landowner waiver, as he is the owner of the land the well is on. Photos were taken to support the findings of this inspection.
1799995694 2014-07-30 Production Wells No Violations Performed an inspection on this well in response to a request from PDC Energy. Upon inspection, it was observed that this well is not currently capable of production. The well consists of a pumping unit with a gasoline motor standing over 4.5 casing and 1.5 production tubing with rods. The well is shut in and no lines are connected to the well head. Brush has grown up around the well and the immediate area has been timbered recently, rendering the access road impassable by vehicle. Production records indicate that the well has never produced. A natural gas odor was detected around the well head, but no leaks were found. No pollution or contamination issues were observed around the well. No identification information is posted at the well, and no tanks were found. RBDMS shows this well as Historic Owner. The last operator to submit production reports for the well was Lakeside, Inc in 2009.
