After searching for 1 hour using the RBDMS coordinates, I was unable to locate the well. The area has been timbered recently, making the area difficult to search in. I will contact Lakeside, Inc and request assistance in finding the well next week. |

Formation Cementing set a 500, 2 stage rip isolation plug from 2587-2087 using 135 sks of 1% gel cement. Stage 1 was set from 2587-2287 using 81 sks 1% gel cement with 3% CaCl, 30 lbs LCM, and 37 bbls freshwater displacement. Stage 2 was set from 2287-2087 using 54 sks 1% gel cement with 2% CaCl, 30 lbs LCM, and 34 bbls freshwater displacement. The 4.5 casing was then pulled to 1699 and a 200 Berea/Bottom of Surface plug was set from 1699-1499 using 53 sks 2% gel cement, 2%CaCl, 30 lbs LCM, and 24 bbls freshwater displacement. Casing was then pulled to 200 and cement was circulated to surface. The remaining 4.5 casing was then pulled and the cement was topped-off to surface and did not drop below 30 below grade. Good circulation was achieved on all plugs. I was accompanied on this job by ODNR DOGRM Emergency Operations Manager Trisha Cartmell. |

The well is currently shut in and flow lines are no longer attached. The well consists of 7 surface casing, 4.5 intermediate casing, and 2 production tubing with a rabbit catcher. The tank battery consists of a 100 bbl tank, a mechanical separator, and a drip. According to Mr. Chuck Ungurean of Lakeside, Inc, this well will be plugged this fall. Lakeside is currently in the process of filing for a plugging permit. GPS was not functioning properly, so no location data was gathered. |

While pulling the 1.5 tubing from the well, the tubing parted at 3190. Jet Wireline Services attempted to run a tool past the top of the parted tubing but was unsuccessful. It was then decided to shoot 8 holes in the 4.5 casing at 3190 to set a plug in an attempt to isolate the Big Lime and squeeze cement into the annulus of the 4.5 casing. Formation Cementing pumped 30 sks of class A cement and displaced the entirety of the plug to 3190 in an attempt to squeeze as much cement as possible through the perforations. The plug was then locked in for a period of 1.5 hrs to ensure it did not travel back up the 4.5 casing. Jet Wireline Services then shot the 4.5 casing off at 2625 and 1 joint of casing was pulled out of the well. Plugging activities will resume on 6/11/2015. |
