0141498934 2014-01-16 Administrative Inspection No Violations Spoke with Mr. J T from Jet Wireline services. He stated that he had perforated the production casing on this well at 1700 ft - 1710 ft. There were 13 shots total. He also stated that his logging tools malfunctioned that day, so they worked off of an old log of the well. He was not able to run a bond log either.
0437165311 2014-02-18 Work Over Reconditioning No Violations Witnessed the beginning stages of well stimulation for this well. Dennis Lang has finished his paperwork and is registered as a domestic owner and owns this well. The formation broke down at 1050 psi. There was no pressure on the annulus. Operation had to be shut down due to equipment issues.
0768853894 2014-01-15 Administrative Inspection No Violations I had received word that this well was being serviced and that the property owners were planning to re-frac this well. The property owners are Mrs. Sharon Nay and Mr. Dennis Lang. I called and spoke with Mrs. Nay at 8:39 am and told her that they could not frac the well because they were not registered owners and the well was not in her or her husbands (Dennis Lang) name as per the Divisions records. Later on at 9:09 am, I recived a phone call from Mr. Dave Kehl. Mr. Kehl was performing the service work on the well. He was calling to notify me that they were going to frac the well. I commended him on his effort to notify me before performing stimulation on a well. I then told him that they could not perform the operation due to the fact that the well owner was not registered and did not have the well under their name. I also told him that it was the well owners fault for not having done this. He then stated that it was an orphan well that they had taken over.
0789947220 2014-01-16 Administrative Inspection No Violations Met with landowners Mr. Dennis Lang and Mrs. Sharon Nay. I observed paperwork they had obtained that showed they were legal owners of the well as per the county courthouse records. I then explained the process of obtaining domestic ownership status. They stated that they had talked to Ms. Erica Freeman in the Columbus office and that she was sending them the pertinant paperwork.

Flowback water is being captured by a 300 barrel tank. House gas is being produced from the 4.5 in casing. Well owners are registered with the state. Well is in compliance.

Later, talked with landowner and she said not particularly interested in keeping it as it is not fraced - doesnt make anything the way it is and couldnt afford to plug it herself, but also not really hurting anything the way it is!

I have talked repeatedly to Sharon Nay, owner of the land, about getting it put in her name. Have called her several times over the last 3 years and still no luck, theatening to put it on the orphan list and up for adoption. According to Stonebridge, who would like to produce the well, legally the well belongs to her. Do we want to do C. O.s?
1284775807 2014-01-15 Administrative Inspection No Violations Spoke with Mr. Jim Rose from Producers Services Corp at 2:42 pm. He stated that Mr. Dave Kehl had called him and called off the frac job. During conversation he also outlined the design of the job.
1390491336 2014-01-15 Production Wells No Violations An inspection of the well site revealed that there was a still a service rig over the well (Mr. Dave Kehls). There was 4.5 in. casing at the surface with a welded well head and a nipple and a valve for the annulus. There was a frac valve installed on the 4.5 in. casing. There was also 1.5 in. tubing ran in the well and set in slips. There was one 500 barrel frac tank on site. There was no personnel on site to talk to. There are no production tanks associated with this well. I was accompanied by UIC Inspector 3 Dave Ball.

1495310638 2014-01-15 Production Wells No Violations Inspected site along with local inspector and it was determined that there was a service rig on site and it appeared that someone was preparing to frac said well.

Well is equipped with 7 casing and 4.5 casing and 1.5 tubing there was a 4 frac valve on casing ,well is located between two ponds and there is a small equipment shed directly north of wellhead.
