There is a noticeable gas odor around the well head and gas can be seen bubbling around the ball seat inside the body of the 2 ball valve. Fort Frye High School employee Mr. Allen Ellis stated that recent homecoming activities for the school had to be cancelled due to safety concerns over the well leaking gas. New photos were taken. |

Mr. Jerry Henderson of Hopewell Oil & Gas stated that he would have this well plugged and abandoned before the start of this school year and failed to do so. The well head has been painted orange in an attempt to make it more visible. There is a childs toy wrapped around the well head, indicating children have been playing at the well. The is no bull plug in the 2 ball valve, leaving it vulnerable to vandalism. The well is 60 from the outfield fence of the Fort Frye H.S. baseball field, 100 from the discus ring, and 130 from the football stadium fence. The well poses a potential threat to public health and safety. I will contact Jerry Henderson and instruct him to install locking bull plugs on the well head and install fencing around the well until it can be properly plugged and abandoned. The well will be referred for a Chiefs Order. Photos were taken to verify the conditions observed on the location. I was accompanied on this inspection by Inspector Brian Sefcik. |

On the 2 valve there was another swage that went down to 1 in. There was also an old piece of flow line on the ground with a hammer union fitting on it. Mr. Ron Rainer stated that the flow line probably went to the boiler room for the high school. There are no tanks associated with this well. It is located in between the baseball diamond and the practice field behind Fort Frye High School. The well has not produced anything since 1987. Hopewell Oil and Gas has only submitted production records for the years 2006 and 2011. They both stated that nothing had been produced. There is no pollution or contamination on the surface. Mr. Ron Rainer and I both smelled gas/ oil vapor during the inspection. This was due to a leak in the 2 in valve. We could hear the gas/vapor escaping. There is no identification on the well. Well must be identified. Well must be placed in production or plugged. Mr. Rainer expressed his concern for the well as a safety hazard for students anybody that may drive a vehicle in that area. The 2in valve on top of the well also needs addressed. |

The well is currently idle and equipped with 4.5 inch diameter casing, reduced with 4x2 inch diameter swage, no production hook-ups, and the valve in the closed position. Gas bubbling can be witnessed above the closed valve. The well poses a general safety concern due to the close proximity of Fort Frye High School football, track, and baseball fields. It will be noted that the well is located within an urban area, source water protection area, and 100-year flood plain. There is no proper identification or safety fence at the wellhead. However, (4) steel pipes have been placed around the well as a barricade. Pictures were taken during the inspection. |

Parkins, found an idle well on Fort Frye school property leaking a small amount of gas into the atmosphere. The well, owned by Hopewell Oil & Gas Development, has been idle for some time and interferes with school activities, due to the fact that it lies in the overflow parking area for several sports events. The well is located 130 east of the football field and track, 100 south of the discus pit and 60 southwest of the outfield fence for the baseball field. The well is also 180-200 northeast of the bank of the Muskingum River. Many times during sports events, spectators are required to park around the well and on this day of inspection, the well was emitting a slight gas odor from about 10 away. The well has a 4 x 2 swage with a 2 x 1.5 ball valve on it, then topped by a 1.5 nipple. You can look down in the nipple and see gas bubbling through the valve, even with the valve closed. Recently, the school has cemented 4 4 steel posts around the well to protect it from being hit. Prior to this inspection, there was a kids toy wrapped around the casing. Records show that Hopewell has ever produced this well. Last production reported was 1987 and records show that it is a Berea well. Pictures taken, SEM is 187. |

Upon inspection, it was observed that this well is not equipped for production and is shut in. The well consists of a 7 surface casing and 4.5 production casing with a swedge to a 2 ball valve. There is no plug in the ball valve to prevent leaks. No lines are connected to the well. There are no tanks associated with the well. No identification information is posted. Mr. Rainer stated that often times there is a strong enough natural gas odor to cause safety concerns, however no odor was present at the time of inspection. The well is located approximately 50 from a high school baseball field, 100 from a football field, and the well is located in an area where cars are commonly parked and could possibly hit it. The well has never reported production. RBDMS records indicate that a plugging permit was obtained for this well by the operator on 10/16/2013. Hopewell Oil & Gas Development will have 180 days upon receipt of this notice to properly plug and abandon the well. |

I attempted to contact the company via the number in RBDMS but the line has been disconnected. The well has had no resolution to any of the issues on it and the well is idle. |
