0592034764 2018-04-25 Production Wells No Violations This well was issued a compliance on 1/12/17 by Inspector Matthew Fulk for well operation causing pollution and contamination. The inspection this day revealed that the areas that Inspector Fulk described in his report have been remediated and vegetation has been established. The areas have been cleaned and there is no longer pollution and contamination on the site and the issue has been resolved.
0906594561 2017-01-12 Production Wells Violations Noted This inspection first brought me to the tank battery, which sits off the road about 100 feet. The tank battery and dike runs perpendicular to the road. The loading line for the 100 barrel steel tank comes through the dike about in the middle. Where the loading line terminates there is a brine scald averaging 5 feet wide and 65 feet long. I used low range Quantabs to preform a quick field test of the suspicious area. The low range Quantabs quickly maxed out. I then repeated this step with high range Quantabs and the test identified chlorides at a level of 1,343 ppm. There was standing water inside of the dike and I also tested this water for chlorides and found there to be 325 ppm chlorides, this reading is from the low range Quantabs as at that level they would provide a more accurate result. The tank battery consists of 1-100 barrel steel tank, separator and a chart. The ID revealed that Huck 1,2,3 all produce into this battery. The line going to the tank from the separator was closed. Inspection of the well revealed that it used a pumping unit with tubing and rods as means of production. There was no motor or engine on the pumping unit. However about 80 feet away there was an electric line coiled up on a pole, indicating at one time this well probably used an electric motor for production. All valves at the wellhead were in the closed position. There was also an area of about 24 circle from the pumping unit that was contaminated. These contamination issues will need corrected.

1773095645 2018-04-25 Production Wells Violations Noted The inspection this day revealed that this well is equipped with a pumping unit, 4 1/2 inch diameter production casing, and production tubing. The valves on the wellhead are closed for production. The tank battery for this well is equipped with a 100 barrel steel tank, a vertical separator, and a meter run with an old chart in it with no date on it. The identification for this well appears to have fallen off the tank and the only legible parts are the company name and emergency contact number. Production reports in RBDMS show that B&E Resources has reported 0 production for 2016 and 2017. The well owner/operator shall submit a written plugging schedule to Brian Sefcik at 5880 Memory road, Zanesville OH, 43701 within 30 days, per OAC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. I spoke with Gary Lang the owner of B&E Resources about this well and let him know I was issuing a compliance notice for this well. He informed me he had already planned on plugging this well this summer and that he has the paperwork filled out and was going to send it in within the next few days to get a plugging permit.
