| 0029913724 | 2013-12-02 | Production Wells | No Violations | It was reported to me at 7:31 PM on 12/1/13 by Mr. Gordy Deer, that there had been an oil spill from the production tank associated with this well. I also received a phone call from Ohio EPA about this well at 9:17 PM that same evening. Mr. Deer stated that there was a hole on the bottom of the tank and about 8 barrels of oil was released. 7 barrels were recovered. Upon inspection I saw an area 15 ft by 15 ft in front of the production tank that was covered in peat moss. Mr. Deer is using the peat moss to absorb the oil. The lease road had a couple piles of lime blocking direct access by vehicle. There was also some oil on the vegetation. Mr. Deer stated that that was where they had drug their hoses across to recover the oil. I spoke with Mr. Deer on the phone, and he stated that he would continue to clean-up the oil on the ground with the peat moss. I then told him that an earthen dike needed built around the tanks. He then stated that he may be moving the tank battery due to disagreements with the landowner that are still under litigation. I will check in with Mr. Deer periodically to see if there are any new developments with that. I gave him a week to finalize the clean-up of the spill. I was accompanied by Mr. Trevor Irwin and Ms. Ricki Knerr of the Ohio EPA. |

The 2 steel line which runs to the mechanical separator was capped earlier this summer after a leak resulted in soil and stream contamination on the property. I spoke with Gordon Deer at 5:23 pm and requested a meeting on the location to discuss plans for fixing the line, replacing the unusable 100 bbl production tank, and building a dike around the tank battery. A meeting is scheduled for next Tuesday at 9:00 am. |

| 1907782696 | 2014-12-18 | Production Wells | No Violations | Performed an inspection on this well in response to a complaint from landowner Mr. Randy Wagner. Upon inspection, it was observed that this well is equipped for production as well as domestic use. The well consists of a 4.5 casing with a swedge to a 2 ball valve and a tee. One side of the tee leads to a production flow line which heads to the tank battery and sales meter. The other side of the tee leads to a pressure regulator and domestic gas meter. All valves are in the open position. Mr. Wagner claims that the production values submitted to ODNR are not accurate and the wells have not sold gas in over 2 years. There is a current chart in the meter run at the tank battery which reads very low to no production, and there is no chart in the master meter for the lease. I spoke with Mr. Gordon Deer of Deer O&G and inquired about the wells, and he stated that he plans on selling gas off of the wells again in the near future after a new tank is brought in and lease conflict is worked out with attorneys. No compliance notice will be issued at this time. |

The valves on the production casing are in the open position. A separator is set in the now shared tank battery with Deer Oil & Gass adjacent Heiner lease. I met with Mr. Gordon Deer on location and he stated he will be setting a new 30 bbl tank which will serve the Hensler lease, but will be located at the Heiner tank battery. There is a current chart in the sales meter which shows gas sales from the Hensler lease. Mr. Deer stated he would contact me when the new tank is set and ID information is posted. Photos were taken of the location and new metering setup. |
