0392971392 2017-01-03 Production Wells Violations Noted I was contacted by ODNR Division of Oil and Gas District Supervisor Eric Conrad about a possible oil release from a flow line. When I arrived on location Ethan Eddie, a contractor for Genesis, was on location and advised me that roughly 2 barrel of oil released from a flow line that connects the well to the vertical separator. The oil that was released traveled 15 feet on top of the ground into a culvert underneath the lease road the leads directly to Eightmile Creek. An estimated one barrel was spilled on the ground and roughly one additional barrel was released into Eightmile Creek at Global Position System coordinates 39.42733 -81.33032. Ethan Eddie had the line shut in and the oil contained prior to my arrival. I witnessed Mr. Eddie put oil absorbent pads down, dig up contaminated soil, and placed oil absorbent pads on the bank of Eightmile Creek where the culvert meets the creek. The Ohio EPA was notified and I made the call to ODNR Emergency Response Program at 12:25 PM. All contamination shall be cleaned up and hauled to a state approved land fill.

1483626796 2017-01-05 Production Wells No Violations The purpose of my inspection is to follow up on the compliance notice numbered -392971392. During my inspection all of the contamination has been remediated. Straw and lime have been placed in the areas where contaminated soil had been removed. An oil absorbent boom remains in Eightmile Creek but there is not any oil sheen visible at this time.

2075919094 2017-01-27 Production Wells No Violations I called Andrew and had him have his pumper meet me out here before going to the B & N 7 well. There was an area in the creek that had an oil sheen on it below the culvert. I explained I would need the pumper to have a boom, diapers and trash bag. That this issue needed taken care of ASAP. There is still free oil in the ditch above the culvert that goes to the creek. The diapers below the culvert by the creek are completely oil soaked and need replaced. Where the water would drip from the culvert was a small standing pool of oil. The sheen of oil on the water in the creek is about 18X3.
