| 1389282084 | 2014-01-09 | Plug / Plug Back | No Violations | On the day of my inspection, I witnessed Formation Cementing break circulation with fresh water then displace thirty five sacks of 14.7 class A cement with 1% Cal and a yield of 1.36, the rig crew pulled 63 joints of inch and a half tubing from the well, the wire line company ran a bond log then shot the four and a half inch production casing in half at 531, the four and a half inch casing was hung up in the red rock at 540, at that point the rig crew pulled six joints of the four and a half so Formation Cementing could set a 300 surface plug. The pit was constructed and lined with ten mill black plastic through the flow ditch and around the well head, I instructed the rig crew to cut the eight and five eights casing off three foot below the surface, then place a cap with the date and I.D. on top of the plugged well and cover with soil. |
| 1867515099 | 2013-09-16 | Production Wells | Violations Noted | An inspection of this well revealed 4.5 in. casing with a swage and a 2 in. valve with 2 in. flowline connected to it. The valve was in the open position. Where the swage threaded into the casing, there was a leak. This was note due to slight bubbling around the connection. Production records show that the well has not produced anything since 1987. The tank battery consisted of one 100 barrel tank. There was about a foot of fluid in the tank. No separator was observed on site. Condition of flowlines is poor. No pollution and contamination was observed. No identification was observed on the well or the tank. Well must be plugged or placed back in production. If the well is incapable of production in commercial quantities as described in ORC 1509.062 it must be plugged. Well must be properly identified. I was accompanied on inspection by Mr. Jeff Sayger of Alliance Petroleum. |
