0526900304 2015-10-14 Plug / Plug Back No Violations On location to witness plugging operations for this well. Matheny Well Service is rigged up over the well and Formation Cementing is on location to set 3 plugs across sets of perforations in the Ohio Shale and 30 Foot Sandstone. For plug #1, 2.375 tubing was set at 3496 and a 200 plug was set inside the 4.5 casing from 3496-3296 using 15 sks class A cement and 15 lbs celloflake as LCM. For plug #2, tubing was pulled to 3270 and a 200 plug was set using 15 sks class A cement and 15 lbs cellflake as LCM. For plug #3, tubing was pulled to 2368 and a 330 plug was set using 25 sks class A cement and 20 lbs celloflake as LCM. Some cement circulated back to surface during the pumping of plug #3, however it was determined that it was light lead cement from plug #1 which migrated uphole during displacement. Tubing was pulled 300 above plug and the hole was circulated clean. Good circulation was established on all plugs. Plug #3 will be tagged and the 4.5 casing will be ripped tomorrow in preparation for plugging the rest of the well out.

1445270217 2015-10-16 Plug / Plug Back No Violations On location to witness plugging operations for this well. Matheny Well Services tagged the top of the previous perforation plug at 2150, indicating it stayed where it was intended to be. The 4.5 casing was ripped at 1635 and 1 joint was pulled. Formation Cementing pumped a 150 rip isolation plug from 1603-1453 with 43 sks class A cement with 2% CaCl and celloflake as LCM. Gel drilling mud was circulated out of the 8.625 surface casing throughout the pumping of this plug. 4.5 casing was pulled to 1306 and a 200 bottom-of-surface plug was set using 59 sks class A cement with 2% CaCl and celloflake as LCM. Casing was then pulled to 200 and a 200 plug was circulated to surface. The remaining casing was pulled and the cement was topped-off to 30 below grade. Cement did not drop. Good circulation was established on all plugs.
2016612524 2015-12-31 Final Restoration No Violations Upon inspection, it was observed that this location is fully reclaimed. The pits are filled, the location is back to grade, production equipment has been removed, the lease road is restored, and there is a full vegetative cover established. Final restoration passed 12/31/15.
