0331390899 2019-01-08 Production Wells Violations Noted On this day, I am conducting an inspection in response to a voicemail on 1/7/2019 from a restricted number informing that a well in a field may have an issue. No callback information for the caller was received. I arrived on location and found a freshly dug drainage ditch through a field, traveling approximately 900 feet, and leading to Cats Creek. It appeared that a flowline was severed during the excavation of the ditch. A rain event had occurred prior to the inspection. Rain water and an oily product of an unknown amount was found within the ditch. The company was notified of the incident, as well as the ODNR One-Call Emergency Notification System. Multiple representatives from Deer Oil and Gas arrived onsite with absorbent pads and booms. The company is currently taking preventative measures and commencing the remediation process. I spoke with Gordy Deer of Deer Oil and Gas, Trevor Irwin of the Ohio EPA, and Ryan Donnelly of the Division of Wildlife. The well is equipped with 4.5 inch diameter casing and 1.9 inch diameter tubing with the production valves closed. It is setup to be produced by means of a pumping unit with rods and an electric motor. The production facility consists of (2) 100 barrel steel tanks, drip tank, vertical separator, and an orifice sales meter with a current chart all contained within an earthen dike. The tank is identified and the production records are current. The facility is shared with permit number 2-5972.

1636085588 2019-01-16 Production Wells No Violations On this day, I am conducting a compliance notice follow-up inspection. Multiple soft booms are currently present within the drainage ditch, and the affected area has been cleaned. I received a message from Gordy Deer of Deer Oil & Gas (1/9/19) stating that the severed flowline has been properly repaired. The prior violation is now resolved.
