0042869624 2017-02-06 Plug / Plug Back No Violations Jet wireline service is on location to tag cement inside the 2.375 inch diameter tubing. Cement was tagged at 3163 feet. The decision was made to shoot the tubing free at 3150 feet. Jet Wireline shot the tubing but it did not come free. Another attempt to shoot the tubing off at 2587 feet was successful. Bowers Pumped at 200 foot rip plug from 2587 feet to2387 feet. The plug consisted of 15 sacks of 15.6 pounds per gallon class A cement. Following the plug Matheny pulled 25 joints of the 2.375 inch tubing.

0934778769 2017-02-10 Plug / Plug Back No Violations Matheny Well Service tagged the bottom of surface casing plug at 685 feet. Due to cold weather the rigs airlines froze up. Once the rig was unthawed Matheny pulled the 2.375 inch diameter tubing up to 100 feet and Bowers pumped a 100 foot plug. The plug consisted of 30 sacks of class A cement weighing 15.6 pounds per gallon. The plug stayed at surface and Matheny pulled the remaining 100 foot of tubing.
1130531397 2011-11-01 Production Wells No Violations My inspection this day due to request from mineral owner. Mr. Russell says he has not received a royalty check in many years from this well - well is idle. He says he has hired an attorney to break the lease. Inspection this day found a casing well with valves open - has the little flow meter they use for low flow wells (it is registering no gas) and also has a Barton meter showing gas sales. The tank has good identification, current chart in meter. Im not sure which meter is used to calculate gas sales. Good dike around the tank. The annual production from 2009 shows 102 mcf and they have not yet turned in production for 2010. If well is selling gas, the mineral owner is not getting paid.

1486656487 2017-02-08 Plug / Plug Back No Violations During my inspection I witnessed Jet Wireline Service run a cement bond log to detect the top of cement on the backside of the 4.5 inch diameter production casing. The top of cement is at 1370 feet. Reading the bond log more in depth we notice some cave-ins on the back side of the 4.5 inch casing and the first free casing collar is located at 1070 feet. Jet Wireline tripped back into the hole and shot the casing off at 1070 feet. Matheny pulled 1070 feet of the 4.5 inch casing from the hole and tripped back in with 2.375 inch diameter tubing to 1138 feet. Bowers pumped a 168 foot plug, 68 feet in the 4.5 inch casing that remained in hole and 100 feet within the 7.625 inch diameter open hole. The cement plug was class A cement weighing 15.6 pounds per gallon.

1645219978 2011-11-01 Production Wells No Violations Inspection this day at the request of the mineral owner. Mr. Russell says he has not received royalties in many years and wants to know if the well is idle. Inspection this day found a casing well equipped with a flowmeter that is used for low flow wells and registering no gas flow. The well is also equipped with a Barton meter (with a current chart) which shows gas sales. Our production database shows this well produced 102 MCF in 2009. Production records have not yet been submitted for 2010. I had an outside operator look at chart and he indicated that neither meter was registering gas. I notified the mineral owner and suggested he hire a meter expert to verify the meter readings for legal purposes. The production tank was properly identified and the dike was in good repair.
