0191787294 2015-01-28 Production Wells No Violations On location to witness oil spill cleanup activities. River Valley Services is on location with a dump truck, flat bed trailer, and mini-backhoe. Mr. Chad Ullman of River Valley Services used the mini-backhoe to temporarily move the 100 bbl tank that overflowed to allow room for contaminated soil excavation. All visibly contaminated soil was excavated from the affected area and was placed into the dump truck to be hauled to B&E Resources yard for composting. Mr. Ullman then measured for a new SPCC compliant dike/tank battery location that would meet ODNR DOGRM spacing requirements. Tree/brush removal was then done to clear the area for the new tank battery location. The old location will have lime and fertilizer mixed in with it before re-seeding to counteract any residual oil product that may still be present from the spill. I will continue to monitor the progress of this cleanup.
0519753533 2015-01-29 Production Wells No Violations Upon inspection, it was observed that the new tank battery location is constructed, the tanks are reset to meet ODNR setback requirements, and a new SPCC compliant dike has been constructed around the tank battery. The separator dump line still needs plumbed back into the water knockout tank and the location needs to be temporarily mulched to prevent soil erosion until seed can be sewn in the spring. I spoke over the phone with Mr. Dave Tuttle of B&E Resources and he stated River Valley Services will have these tasks complete by the end of the week. I will check to ensure this was done next week before closing the compliance notice.

1422227540 2015-01-25 Production Wells No Violations I received a call at 3:40pm on Sunday January 25, 2015 from Regional Supervisor, Dan Goins, about an oil spill inside a dike adjacent to the property owned by Kelly Haas. I called and talked to Kelly and she stated that she was unable to get through to the operator, Gary Lang, about the incident. I attempted to make contact with the operator and left a message. I arrived on location around 5:15pm along with the Washington County Inspector, LJ Parkins, to find the Field Superintendent, Dave Tuttle and attendant, in the process of cleaning oil out of the dike with a vac truck. He stated that the well had pressured up and caused the tank to overflow about one barrel of oil. The well is shut in at this time. The oil was contained inside; however, cleanup operations, foot traffic, and surface runoff has caused several areas of oil sheen outside of the dike. The operator had diapers and brought more to the location to adsorb the oil in and outside of the dike. Inspector LJ Parkins will take over the situation until the operator is back in compliance.
1422235336 2015-01-25 Production Wells Violations Noted I received a phone call from Southeast Region Supervisor Dan Goins at 3:40 pm regarding an oil spill at a tank battery adjacent to Ms. Kelly Haas residence on Plum Run Rd. near Waterford. I responded to this call and arrived on location shortly after Inspector Jake Glascock at 5:25 pm. Upon arrival, B-E Resources Field Superintendent, Mr. Dave Tuttle and a roust crew member, Mr. Cory Offenberger, were in the process of using a vac-truck to suck up free oil product from the tank battery. Mr. Tuttle informed me that the spill was caused by the well kicking off and flowing more oil than the tank could hold. The well is now shut in. The earthen dike surrounding the tank battery contained the entirety of the initial spill, however, a slight sheen could be seen in areas outside the dike due to foot traffic tracking some oil out of the diked area. After the majority of the free product was sucked up and temporarily pumped into the 100 bbl brine storage tank, the crew placed oil absorbent pads throughout the tank battery to help absorb as much free product as possible from the gravel and soil. A larger roust crew will be on location on Monday morning with a mini backhoe to scoop out contaminated soil and an excavator to temporarily move the overflowed tank so soil can be removed from underneath it. B-E Resources will have until 2-04-2015 to have the contaminated soil removed, the location limed/fertilized, the containment dike built back to SPCC standards, and the location mulched to prevent excessive soil erosion until it can be seeded in the spring. Pictures were taken to support the findings of this report. I will continually monitor cleanup progress until it is completed.
1422301229 2015-01-26 Production Wells No Violations B&E Resources roust crew on location working on cleanup activities for oil spill that took place on 1/25/2015. A mini-backhoe is on location digging a small pit to flush remaining pooled up free oil product into. After flushing the affected area, the vac truck was used to suck the flushed free product out of the temporary pit. Due to deteriorating road conditions, this was all the cleanup done for the day. Work will resume Tuesday morning.
1422392497 2015-01-27 Production Wells No Violations No crew was on location upon my arrival today. I called Mr. Dave Tuttle from B&E Resources and he stated that Chad Ullman from River Valley Services will be on location tomorrow to temporarily remove the tanks and excavate the contaminated soil. It was also observed that the tank battery does not meet spacing requirements and will have to be relocated to a location at least 50 from Plum Run Rd. I was accompanied on this inspection by Southwest Region Supervisor Dan Goins.
1423062017 2015-02-03 Production Wells No Violations Upon inspection, it was observed that the conditions of the compliance notice have been met. The new tank battery location meets ODNR spacing and setback requirements. The new containment dike is SPCC compliant and all contamination has been removed, and the location has been mulched to prevent erosion until it can be seeded in spring. The compliance notice dated 1/25/15 can be closed.
