0039166003 2016-06-27 Production Wells No Violations Upon inspection, it was observed that vegetation is beginning to grow on areas previously affected by brine release. Marilyn Haynes has yet to supply new production tanks so the wells can be put back into production. New photos taken. I was accompanied on this inspection by DOGRM Emergency Ops. Coordinator Nathan Lane.
0068225517 2016-08-18 Production Wells No Violations Upon inspection, it is observed that cleanup operations on the previously brine scalded area is complete. Vegetation is now growing on the affected area and no other contamination is present. The well is not yet plumbed into a production tank and it is not being pumped. The well is providing domestic gas to the Marilyn Haynes residence. Ms. Haynes states she has purchased tanks to set for the well and others on her property, however there is currently a dispute with the contractor who is hired to set them and build containment dikes. No identification information has been posted at the location as of yet. I am accompanied on this inspection by DOGRM E.R. Coordinator Nate Lane.
0175636331 2016-04-25 Production Wells No Violations On this day I stopped by the site to assist the County Inspector with a situation that has occurred from a 100 barrel production tank that has failed near the bottom seam and released a undetermined amount of oil into a stream adjacent to the production tank, the product then traveled down the stream approximately 1,387 into the North Mill Fork Creek. There were crews on site from various production companies along with Ohio EPA representatives and ODNR Emergency Response and field staff. Multiple collection points by means of vacuum truck and siphon dams were set up retaining the product to the direct area. Crews will remain on site until all product is recovered. No earthen dike was observed present and no well identification was present. The lead inspector will issue violations and follow up with correcting the well location coordinates with the division permitting staff. The well was observed to be equipped with 6.625 production casing -2 production tubing -pumping rods and a production tree with a 1 vent packer string present and both the tubing and 1vent packer string fitted with production valves in the open position selling into production flowlines.
0322085347 2016-05-10 Production Wells No Violations Upon inspection, it was observed that the rolloff box of contaminated soil has been removed from location. The affected area has been re-graded with clean soil, limed, seeded, and mulched. The well is not being pumped, but is still supplying domestic gas to the Haynes Residence. The well will not be pumped until it is plumbed into a production tank. I will continue to monitor this location for compliance. Photos taken.
0507574610 2018-02-09 Production Wells No Violations This well was issued a compliance notice for well operation casing pollution and contamination, failure to legibly identify well, and no SPCC dike/or failure to keep dike free of water or oil by Inspector Lynn Parkins on 04/27/16. The inspection this day revealed that this well is equipped with a pumping unit, 4 1/2 inch diameter production casing, and production tubing. There is a valve open on the wellhead for production. The tank battery consists of a 100 barrel steel tank with a proper earthen dike built around it. There is no legible identification on either the well or the tank. Along the creek there are still used booms from the original release that happened. I spoke with Roger and Marilyn Haynes about this well and 66584. They claim that they had identification posted and was unaware that it is now gone. They believe someone may have stolen it from their wells. I informed them that they can paint it on the pumping units and the tank so it would be harder to remove or they can post new stickers with the pertinent information. They also were unsure about how to properly dispose of the contaminated booms. They have told me that they have spoken to John Schneider of Stonebridge before. I asked if they still had his number and if so to contact him to see if he could help them with disposing of the booms. They informed me that they have been unable to pump these wells because the motors on the wells have seized but have been receiving house gas to run appliances off of.
0676916826 2016-04-27 Production Wells No Violations Cleanup crews are staging rolloff boxes and excavation equipment to begin removing brine contaminated soil from this well along with soil affected from the oil spill. The 100 bbl production tank will be inspected for leaks and replaced if necessary.
1060692896 2003-08-11 Not Drilled No Violations Inspection this day could not find a well ever drilled. Talked with current landowner and previous landowner and they both say well never drilled. Must be a mistake. Upon further study, believe 7221 and 7227 are switched. Landowner must be producing 7224 and 7221, not 7227 as he thinks. Will do additional inspection to verify. Computer shows 7227 not drilled.
1063054239 2003-09-08 Production Wells No Violations Well in production with no violations, owner is Roger Haynes - says went thru court to obtain it. Gave hime paperwork and he will complete to become an operator

1626416562 2016-05-16 Production Wells No Violations Upon inspection, it was observed that the well is not yet plumbed into a tank, but is still supplying domestic gas to the Haynes residence. Brine affected areas which have been limed, seeded, and mulched have yet to begin growing grass. No ID posted yet. Photos taken.
1979217534 2016-05-20 Production Wells No Violations On location with Nate Lane and Trisha Cartmell to check status of brine contamination cleanup at this well. Grass and clover is beginning to grow on seeded areas. The well is not yet plumbed into a production tank. Photos taken. A meeting has been set up for 10:00 am on 5/24 with Marilyn Haynes and Ronnie Earley to discuss building containment dikes and putting the lease back in compliance.
2024934336 2016-04-26 Production Wells Violations Noted Performed an inspection of this well in response to an oil spill at another well on this property. Upon inspection, it was observed that this well is currently in production. The well consists of a pumping unit standing over 4.5 production casing and 1.5 production tubing with rods. Valves on the tubing and casing were in the open position. The well is producing domestic gas for well owner Marilyn Haynes. No ID information is posted at the well or tank. The tank battery consists of a 100 bbl production tank. The tank was full and overflowing brine and oil at the time of inspection. There is no containment dike around the tank. There is a large brine scald area originating at the tank and encompassing an area 50 long by 25 wide. Cleanup crews who were working on the oil spill in Mill Creek were notified of the overflowing tank and they acted quickly to shut the well in. A J. Eddy Oil Gathering truck was brought onto location to suck the tank empty. The production tank currently sits 15 from the well, which is in violation of DOGRM tank spacing requirements. The well owner will have 14 days from the issue date of this notice to have the following completed: 1.) Excavate all contaminated soil from the location and dispose of it at an OEPA approved landfill; 2.) Re-set the 100 bbl production tank at least 50 away from the well head and construct an earthen containment dike around it to meet SPCC guidelines; 3.) Post legible and correct identification information at the well or tank.
2108834800 2016-05-02 Production Wells No Violations Upon inspection, it was observed that contaminated soil has been excavated from the location, the 100 bbl production tank has been removed from location, and the affected area has been re-graded, limed, seeded, and mulched. The well will remain shut-in until it is plumbed into a competent production tank.
