| 0175636331 | 2016-04-25 | Production Wells | No Violations | On this day I stopped by the site to assist the County Inspector with a situation that has occurred from a 100 barrel production tank that has failed near the bottom seam and released a undetermined amount of oil into a stream adjacent to the production tank, the product then traveled down the stream approximately 1,387 into the North Mill Fork Creek. There were crews on site from various production companies along with Ohio EPA representatives and ODNR Emergency Response and field staff. Multiple collection points by means of vacuum truck and siphon dams were set up retaining the product to the direct area. Crews will remain on site until all product is recovered. No earthen dike was observed present and no well identification was present. The lead inspector will issue violations and follow up with correcting the well location coordinates with the division permitting staff. The well was observed to be equipped with 6.625 production casing -2 production tubing -pumping rods and a production tree with a 1 vent packer string present and both the tubing and 1vent packer string fitted with production valves in the open position selling into production flowlines. |

Along the creek there are still used booms from the original release that happened. I spoke with Roger and Marilyn Haynes about this well and 66584. They claim that they had identification posted and was unaware that it is now gone. They believe someone may have stolen it from their wells. I informed them that they can paint it on the pumping units and the tank so it would be harder to remove or they can post new stickers with the pertinent information. They also were unsure about how to properly dispose of the contaminated booms. They have told me that they have spoken to John Schneider of Stonebridge before. I asked if they still had his number and if so to contact him to see if he could help them with disposing of the booms. They informed me that they have been unable to pump these wells because the motors on the wells have seized but have been receiving house gas to run appliances off of. |

Upon inspection, it was observed that this well is currently in production. The well consists of a pumping unit standing over 4.5 production casing and 1.5 production tubing with rods. Valves on the tubing and casing were in the open position. The well is producing domestic gas for well owner Marilyn Haynes. No ID information is posted at the well or tank. The tank battery consists of a 100 bbl production tank. The tank was full and overflowing brine and oil at the time of inspection. There is no containment dike around the tank. There is a large brine scald area originating at the tank and encompassing an area 50 long by 25 wide. Cleanup crews who were working on the oil spill in Mill Creek were notified of the overflowing tank and they acted quickly to shut the well in. A J. Eddy Oil Gathering truck was brought onto location to suck the tank empty. The production tank currently sits 15 from the well, which is in violation of DOGRM tank spacing requirements. The well owner will have 14 days from the issue date of this notice to have the following completed: 1.) Excavate all contaminated soil from the location and dispose of it at an OEPA approved landfill; 2.) Re-set the 100 bbl production tank at least 50 away from the well head and construct an earthen containment dike around it to meet SPCC guidelines; 3.) Post legible and correct identification information at the well or tank. |
