Significant progress has been made in attempt to gain compliance of the previous lease road issue. Mr. Hamilton is utilizing a secondary lease road for access to the well. An erosional issue on the secondary road still needs to be addressed. I spoke with Mr. Hamilton on 10/26/2018, and he indicated that he will address the spot in question. I also spoke with landowner, Ms. Kuehne, on 10/26/2018 and she informed me that gas was reconnected to her cabin on the property. A follow-up inspection will be conducted before resolving the issue |
| 1536603384 | 2018-09-10 | Preliminary Restoration | Violations Noted | On this day, I am conducting an inspection in reference to Complaint #5955 with regard to an odor coming from the well. I spoke with landowner Lori Kuehne on 9/10/2018, and she informed me that there had been a slip within the lease road, which could be the potential reason for the odor. Ms. Kuehne also indicated that she hasnt received gas to her cabin in several months, possibly due to the slip, as well. Upon inspection, I found the slip within the lease road and substantial erosion has occurred, but it is unknown when this originated. The lease road is subject to foot traffic only, and found to be impassible by vehicle. There is no indication of any odor of gas within the vicinity of the slip. My Ventis Pro4 gas detector doesnt indicate any signs of gas, either. I found the well equipped with 4.5 inch diameter casing with the production valve open, and 2.375 inch diameter tubing with the production valve closed. It is setup to be produced by means of pumping unit with rods and a gas engine. It will be noted that no drivebelt is present, and no counterweights are attached to the unit. There is also no indication of gas leaks at the wellhead. The production facility consists of (1) 100 barrel steel tank within an earthen dike. The well is properly identified and the production records are current. Further investigation along the roadside (Sacket Run, T67), near the entrance to Ms. Kuehnes cabin, found a severed 1 plastic flowline within the drainage ditch, which appears to have been excavated sometime recently. Ms. Kuehne indicated that the gas to her cabin was out prior to the excavation of the ditch. No gas appears to be released from the lines during the inspection. Multiple pictures were taken during the inspection. An attempt to contact Leonard Hamilton was made on 9/11/2018 at 10:10 am, and a message was left with his wife, requesting him to return my call.
