0444327340 2013-07-09 Production Wells Violations Noted I was not able to access the well head due to its location in the middle of a corn field. The tank battery consists of 2 210 barrel tanks with a drip and a meter run. The meter had a new chart on it. There was no pollution and contamination at the tank battery. Spoke with landowner on the site. He stated that he had been maitaining the lease road because he also uses it. The company has not maintained it. Production records for this well state that the well has not produced anything in 2011 or 2012. Well must be put back in production.

1084484708 2014-07-28 Plug / Plug Back No Violations Witnessed plug back activities at this well location. Upon arrival, Altier Brothers was rigging up over the well. Formation Cementing arrived on location at 10:30 am. 1.5 tubing was set in the well to 6058 to set the bottom plug. 15 bbls. of fresh water was ran ahead of the plug to ensure the tubing was clear. The bottom plug was set using 30 sks of class A cement with 20 lbs cottonseed hulls as LCM. The plug was displaced with 14.4 bbls. of fresh water. Tubing will be tripped out of the hole, the old 4.5 casing will be shot-off and pulled, and a 200 rip plug and 200 Big Lime plug will be set tomorrow afternoon.
1406674689 2014-07-29 Plug / Plug Back No Violations Arrived on this location at 1:45 pm to witness plug back operations on this well. Jett Well Service shot the 4.5 casing off at 4560 and the casing was pulled up to 4432. Formation Cementing circulated on the well and pumped a 200 Big Lime/rip plug from 4434-4234 using 58 sks of class A cement with 30 lbs cottonseed hulls as LCM. The plug was displaced with 67.3 bbls of fresh water.
1702526196 2014-07-30 Plug / Plug Back No Violations Visited this location to witness the cementing of new 4.5 production casing. Upon arrival, Mr. Gary Lang stated that 45 joints, totaling 1898 of 4.5 casing had been run into the hole. Formation Cementing began pumping and established good circulation. They then pumped 97 sks of class A cement through the casing shoe to bring the cement 100 into the 8.625 surface casing. 30 bbls of displacement was used to land the wiper plug. Once the wiper plug landed at 300 psi, Formation Cementing pressured up to 1150 psi and closed the valves on the cement head. The pressure bled off to 1125 psi over a period of 5 minutes, indicating a good pressure test. Mr. Lang stated that a bond log will be ran on the well to verify the top of cement.
1800218370 2014-08-12 Production Wells No Violations Visited this location to witness well stimulation activities. Upon arrival, Producers Service Corp. was rigging up and preparing for the hydrofrac job. I was informed by Mr. Gary Lang that the job would commence around 12:00 pm. I had to leave the location at 11:30 am to witness a plug job at API: 34-167-29224-00-00. I spoke with Mr. Lang via phone call at 4:19 pm and he stated that the hydrofrac job was successful and the Berea Sandstone broke down at 1750 psi. I will pick up the stimulation paperwork this week.
