0324447124 2015-08-25 Production Wells No Violations Received call from complainant Toby Metts at 1:07 PM on 8/21/15 stating that this well is still leaking gas. Upon inspection, it was observed that the 4.5-8.625 annulus is leaking gas and H2S is present. H2S levels of 60+ ppm were observed at the well head. I called Jordan Jones of G-R Contracting Inc. and left a voicemail notifying him of the situation and am waiting for a response. I was accompanied on this inspection by Inspector Brian Sefcik.
0463145596 2010-04-13 Production Wells No Violations Inspection this day found rabbit well producing, still fairly strong odor of rotten eggs - probably had H2S leak around wellhead, even after they have worked on it. Good IDs, good dike, current chart in meter.
0885445316 2015-07-12 Production Wells No Violations Received a complaint from Mr. Toby Metts of Macksburg at 2:58 pm regarding a gas leak at the well head of this well. I arrived on location at 4:40 pm to inspect the well. Upon inspection, it was observed that a 2 nipple connected to the 4.5 well head is badly corroded and has a significant gas leak. The well tender has attempted to fix the leak in the past with a spring clamp, but was unsuccessful. I contacted Mr. Jordan Jones of G-R Contracting and notified him of the leak. Mr. Jones stated he would have a roust crew on location first thing Monday morning to address the leak.

1476728813 2016-10-17 Production Wells No Violations The purpose of my inspection today is to respond to a complaint about smells of Hydrogen Sulfide Gas (H2S). This well is equipped with 8.625 inch diameter surface casing, 4.5 inch diameter production casing, 1.5 inch diameter tubing, and a plunger lift system. During my inspection a small smell was detected but the Sperian Gas meter picked up zero readings of any H2S levels. The well is producing through the 1.5 inch diameter tubing and the chart meter is dated 9/27/2016. I called the complainant Toby Metts and stated to him the findings of my inspection.

1921224989 2015-07-30 Production Wells No Violations Following up on Mr. Toby Metts complaint of a leaking well head at this well. Jordan Jones of G-R Contracting called stating they would be servicing this well today and would be flaring off gas until the well dies down to fix the leaking well head. Upon arrival, G-Rs roust crew was on location and flaring. H2S levels were low at 2-3 ppm. The crew stated that once the well dies down, they will completely re-plumb the well head and stop the leak. I will re-inspect the well next week to verify if this has been done.
