- I said that would be the latest he could wait. Should have already been done.

0855026821 2009-10-14 Production Wells No Violations this well producing with jack, location has been reclaimed. This is one of numerous wells that was a PB - has been coded as such yet still shows up in activities as FRD. Computer needs fixed for this error.

-- probably only a few days or a week but could be up to month ago, but regardless, trees and vegetation already showing signs of dying. Path of water is approximately 10 wide at top, channelling down about 100 and ending in the woods. At top where tanks sat, ground has been smoothed with dozer, covering up some of the most contaminated soil. Soil samples taken and will be analyzed. This recompletion a joint-venture and Jim Mills, operator of well and Carl Heinrich - he and his men engineered and supervised the work and are responsible for the draining of the tanks.

1287495775 2008-04-23 Preliminary Restoration No Violations Inspection this day found location has not been seeded and mulched. Area down the hill where they had dumped the flow-back water is growing back - soil samples taken last Fall showed soil contamination for an area about 6 wide and 100 long. One big cedar tree is brown and dead but that could be attributed to root damage with the dozer when they tried to cover up ground where dumped water. Called Jim Mills and told him it was past deadline to reclaim location and that we had test results showing flow-water had been dumped. Told him at this time, we would not fine him due to new growth but I wanted to make him aware that it had happened. He seemed surprised and said Carl had been an operator for a long time, so it couldnt possibly be true? Took more pictures and told him he might have to bring a dozer back in to reclaim the hill as badly eroded, due to no vegetation. Will not give NOV at this time - believe he will fix problems.
2069187827 2007-08-29 Production Wells No Violations Inspection this day found well had been refracted in Ohio Shale, frac tanks still in place, new tank battery being set with 2 tanks, separator being hooked up this day, good IDs on tank, rig still on well. No signs that water has been dumped.
