0383698800 2010-08-25 Production Wells No Violations Inspection this day found service rig still on location, well with jack ready to produce, stringing sales line right now. Frac water still in tanks, lined pits. Good IDs at well, tanks not set yet.
0519106238 2011-09-02 Production Wells No Violations 2 large frac tanks still contain flow-back water - owner wanted it tested to see how salty, could maybe put on ground if fresh? Tests show too briney to dump, must be hauled to disposal or could put down annular disposal on lease when it gets going, for a total of 10 bbl/day. Owner says was going to use it over again and thats why he has saved. Pit down below all filled in, seeded and mulched. Operator has submitted Landowner Waiver to keep pond that he used to frac out of - says he has added fish to it. Will approve.
0632237414 2015-06-22 Production Wells No Violations Upon inspection, it was observed that this well is currently in production. The well consists of a pumping unit standing over 7 surface casing, 4.5 production casing, and 2 production tubing. The well is producing on the 4.5 casing as well as 2 tubing. The tank battery for the well is shared with P#29710 and consists of a 100 bbl production tank, mechanical separator, and containment dike. There is a pit within the containment dike where rain water has accumulated. I tested the water in the pit with a Hach Quantab and found the chloride level to be 2457 ppm, indicating residual chlorides present in the soil from brine dumping in the past. There is no indication of active brine dumping on the location, as all produced fluids are being sent to the production tank. I will check this lease again in drier weather to see if the water in the containment dike persists.
0874312475 2010-11-09 Preliminary Restoration No Violations
1349967604 2012-10-09 Production Wells No Violations My inspection this day is routine - found pumping unit actively pumping, good identification, could use more dike. The area around the well and tanks has been reseeded and mulched recently and reclamation should be complete.
1785551321 2020-4-28 0:00: TD No Violations 1.10
1797134369 2010-08-03 Completion Testing No Violations Inspection found well flowing back into drill pit, barely cracked. Getting ready to run tubing, sand pump.
2054468203 2011-08-17 Annular Disposal No Violations Inspection this day due to pressure test to approve for annulur disposal. Dan Jones pressured up well to 460# with nitrogen - after 15 min, dropped only 4#. Passed test.
2097135386 2010-07-26 Completion Testing No Violations Inspection this day found loggers on well, perforating in the Berea - 6 total. Pit is still open, liner good on most of it but told them if not closing it in now, to pull up liner on all sides. Service rig on location, should frac in couple weeks.
2119117228 2011-08-12 Production Wells No Violations Inspection found brine now being stored in pit, about 10 x 30 in size, behind location. Pit is about 60 lower in elevation and was not found on first inspection. Owner/operator told me this pit was for emergency use and was empty. It is currently full with frogs jumping in upon my arrival. Kill area below the tank battery to this pit indicates a release. 8/15 - Called Bobby and told him to get pit emptied, filled in and to start removing te fluid from the pit and dispose of it properly. This is Berea well that does not meet Exempt Mississippian production. Will monitor all his operations closely. 8/24 - Bobby says pit is closed, seeded and mulched. 9/2 - Pit is closed, covered up and smoothed, seeded and mulched. NOV Resolved.
