Received phone call from Mr. Tim Neill at 1:16 pm stating that they were shutting the frac down and going into remedial mode.

1252182311 2013-08-11 Drill / Deepen / Reopen No Violations Witnessed well stimulation activities for this well. The first stage broke down at 7218 psi. Annular pressure was encountered during the fracturing process. Pressure went up to 1098 psi. At that point, the pumps were shut down and the pressure was monitored. It stayed around 900 psi. Then the company blew it down. Company then started to flush and finish the stage. The annular pressure walked up to about 450 psi twice, and was blown down twice. After that, the rate was dropped to 71.1 barrels per minute and the annular pressure stabilized at 422 psi. They then decided to continue the stage and started adding sand. The annular pressure then dropped consistantly after that.

Then, when they tried to run a log, the tools sat down at 130 ft. The surface casing job was not able to take place.

-167-2-9720-01-00 at 7:12 pm. Mr.Harold Roper told me that they had a blowout and a drilling mud spill. When I arrived on location the area around the drilling rig had been cleaned up by scraping off the top layer of aggregate off of the pad. The bulk of the drilling mud was contained in the cellar. It was reported to me that approximately 100 barrels of drilling mud had left the well. There were vacuum trucks and super suckers already on location cleaning up the cellar. The rig crews were also cleaning up the rig using pressure washers and steam jennies. The amount of fluid that made it outside of the drilling pad containment was minimal (less than 1/2 barrel). It was in the form of a mist. It was blown by the wind toward the Southwest corner of the drilling pad. It landed in a 15 foot zone from the edge of the pad in an area 50 foot long. Once again, it was a fine mist and it landed on the grass.

1375819165 2013-08-05 Drill / Deepen / Reopen No Violations Well has frac head installed. There are two sand vats on site from Halliburton. I was accompanied by permitting geologist Rick Cisler, Kenny Brown, and intern Andrew Destocki.
1376931602 2013-08-12 Completion Testing No Violations I witnessed part of 2nd stage of Frac job. Company had Halliburton place 1500 gallons of 7.5%Hydro Chloric acid pumped down at perforations for breakdown. The breakdown pressure was at 4146 psi. Highest annular pressure was at 600 psi. The concentration of the frac gel was not quite right and the job sanded off. The company then decided to flow back and go back into the job which this allowed the 30/50 sand to relax and started again. They were at 2.5lb/gal with the sand concentration and at 3 barrel per minute. With 1334 sacks of 30/50 sand away the rate was at 4.8 barrel /minute. Then at 7 barrel per minute the casing pressure is at 9918 psi. At 2:05 am the rate is at 3.9 barrel per minute at 10,940 psi. The annular pressure is at 334 psi. At 3;35 am pumping at rate 3 barrel / minute and pressure is 7650 psi. Shut down. There is a direct relationship between casing pressure when pumps shut down and annular pressure. Pressure up again to 11,365 psi and 581 psi on annulus. Company shut down Halliburton. Company contact is Chris Kruger.
1377083533 2013-08-12 Administrative Inspection No Violations At 4:00 am I received a phone call from Inspector Pat Shreve. He stated that during well stimulation, they had sanded off. He also stated that the annulus pressure was rising during pumping. At 8:17 am, I called Supervisor Dan Goins and told him of this. He told me to shut the operation down. At 8:25 am, I called Tim Neil, engineer for Triad Hunter, and told him to cease stimulation operations.

1514490316 2017-12-28 Completion Testing No Violations Status check inspection to document the well status. The well drilling process was previously completed and 5.5 diameter production casing ran in the well bore. There was problems previously with the production cement job and the 5.5 production casing was attempted to be squeezed with the company reporting a top of cement on the annular of the 5.5 casing at approximately 11,400. The well was previously completed in 250 intervals with 14 stages total completed with a 400 set back from the top of cement. The completion plugs were drilled out and the well flow tested for 2 weeks. There is a solid completion plug set in the 5.5 diameter production casing at 7,000. The company will remove the plug and make a clean out run to bottom and put the well in production. There is 2 other wells on the pad that is being stimulated and all annulis on each well is being monitored electronically. There is flow lines attached to the annulis and running through a choke valve.
1531749242 2018-07-16 Production Wells No Violations Status check inspection found the well now in production. The well is equipped with a lower A section attached to the 13.375 diameter casing string with a riser pipe and both a manual and electronic transducer gauge on the 13.375 well head section, the gauge reads 300 pounds per square inch (psi). The upper B section of the well head has a riser pipe and pressure gauge present monitoring the casing pressure of the 9.625 diameter casing string with 450 psi present on the manual gauge. The 5.5 diameter casing string well head section shows 697 psi and the tubing well head section production flow line shows 450 psi. The well head is equipped with an emergency shut down valve that is controlled both remotely and by multiple electrical emergency shut down sites on the well pad. The well is selling through a sand trap system before entering the gas processing unit followed by a contact tower and glycol heater. The final gas sales point is a Blue Racer midstream sales point on the North side of the well pad. Also present is a production fluid facility consisting of 4 steel construction tanks of 400 barrel fluid volume, the tanks are situated inside a polymer lined steel containment dike area. The company has installed a low pressure vapor recovery system on the production equipment.

The wire line company set a frac plug @ 14,537 and perfed 48 shots @ 14,506 to 14,308. At 18:30 Pm the frac ball was dropped and chased to bottom with H20 and once seated 2 slugs of 7.5% HCL at a capacity of 800 gallons each was spotted with the pressure and rate gradually increased to a maximum of 8109 psi and 35 barrel per minute with the annular pressure increasing with the pump rate and pressure to 500 psi. At 19:00 Pm the job was shut down due to a leak at the well head. Off site at 20:30 per supervisor Dan Goins.
