0342203634 2016-06-08 Preliminary Restoration No Violations The purpose of my inspection today is a status check on a preliminary restoration. During my inspection the well is equipped with 8.625 inch surface casing, and 4.5 inch production casing, 4.5 inch to 2 inch swage nipple, and a ball valve in the closed position. Four frac tanks are on location. The preliminary restoration cant be passed until the frac tanks are removed.

0469423006 2019-04-17 Production Wells Violations Noted On this day, I am conducting a preliminary restoration inspection. I found the well equipped with 4.5 inch diameter casing and a 4x2 inch diameter swage with the 2 inch valve closed. There are no flowlines attached at the wellhead, and appears to be idle. Additionally, the well was drilled in 2014 and has yet to be placed into production. There are (3) frac tanks, not in use, present at the production facility. I spoke with Mr. Larry Lang on 4/22/19, and he stated that he plans to complete the well by Fall of 2019, and the frac tanks will be used in the completion process. The well owner/operator shall submit a written plugging schedule to Tom Thomas at 5880 Memory Road Zanesville, OH 43701 within 30 days, per ORC 1509:13(B)(7) and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied
1395757482 2014-03-25 Casing String Witnessed No Violations Witnessed the cementing of the surface casing. Circulation was not established at the start of the job. Gel was pumped down ahead of the cement. 60 sacks of cement was pumped. Cement came up to the 3rd joint down. After that, cement was pumped from the top. It took 10 more sacks to fill it up. It stayed up after that.
1395791402 2014-03-25 Drill / Deepen / Reopen No Violations Witnessed the pressure test performed on the surface casing 8 hours after it was cemented. The test started at 400 psi., ran for 15 minutes and ended at 375 psi. It passed the test.
1395928497 2014-03-27 Casing String Witnessed No Violations Witnessed the cementing of the 4.5 in casing string. 1889 ft. of 6.25 in hole was drilled. 1878 ft of 4.5 in casing was ran. Good circulation was achieved. After wiper plug landed, 1200 psi of pressure was used to test casing. It was held for 5 minutes. Pressure did not drop.
1542235403 2014-03-24 Casing String Witnessed No Violations Witnessed the cementing of the conductor casing. 64 ft. of casing was ran. Conductor landed in bedrock and the hole was dry. I consulted Inspector Pat Shreve on the matter. We decided that it would be adequate because of the depth of the next string of casing. I consulted Geologist Steve Opritza on the casing diameter change from 11.75 in. to 9.625 in. for the conductor and he approved it. He said he would follow-up with an e-mail. The operator will be changing the diameter for the surface casing as well. It will go from 8.625 in. to 7 in.
