0089396768 2016-06-08 Preliminary Restoration No Violations The purpose of my inspection today is a status check on a preliminary restoration. During my inspection the well is equipped with 8.625 inch surface casing, 4.5 inch production casing, 1.5 inch tubbing, rods, and a pumping unit. The 1.5 inch tubing valve is opened and the 4.5 inch production casing valve is opened. The production equipment consists of three 210 bbl. tanks, 2 vertical separators, and 2 chart meters with chart dates reading 6/7/16. All production equipment will be shared between 29752, 29739, 29740, and 29738. Four frac tanks are on location. The preliminary restoration cant be passed until the frac tanks are removed.
0275215300 2019-04-10 Preliminary Restoration No Violations I was on location today to do the preliminary inspection on this well. The well is in production and meets all requirements.
0378334118 2014-04-22 Drill / Deepen / Reopen No Violations Obtained accurate GPS coordinated for well. Pit has not been closed but restoration had started to take place. Spoke to equipment operator on site. He stated that the water truck was supposed to be in today to suck the pit out. Accompanied on inspection by inspector LJ Parkins.
1397033548 2014-04-09 Casing String Witnessed No Violations Witnessed the cementing of the surface casing string. Good circulation and good returns were observed at the surface. No chlorides were detected in the mix water. After wiper plug landed, the cementers pressured up to 700 psi on the casing and held it for 5 minutes. It lost no pressure. The backside of the casing burped after cementing and the cement fell 20 ft. It was topped off using buckets of cement. Cement stayed at the surface after that.

1686197125 2019-04-17 Preliminary Restoration Violations Noted On this day, I am conducting a preliminary restoration inspection. I found the well equipped with 4.5 inch diameter casing and 1.9 inch diameter tubing with the production valves open. It is setup to be produced by means of a pumping unit with rods and an electric motor. The production facility consists of (3) 210-barrel steel tanks, (2) separators, and (2) orifice sales meters with current charts all contained within an earthen dike. The facility is shared with permit numbers 9739, 9740, and 9752. The tanks are properly identified and the production records are current. Erosional issues within the lease road will need addressed in accordance with ORC 1509.072 (A). I spoke to Mr. Larry Lang on 4/22/19 to inform him of the issue. It will be noted that there are (3) frac tanks near the production facility that will be used in the completion process of permit number 9738.
