0242699448 2016-06-06 Preliminary Restoration No Violations The purpose of my inspection today is a status check on a preliminary restoration. During my inspection the well is equipped with 8.625 inch surface casing, 4.5 inch production casing, 1.5 inch tubbing, rods, and a pumping unit. The 1.5 inch tubbing valve is opened and producing into the same tank as #29750. Identification is posted at the wellhead. The site has been restored with no erosion. Location has met 1509.072 A requirements.
0315813232 2014-10-07 Drill / Deepen / Reopen No Violations On location at 6:00 pm to witness surface casing cement job. Monroe Drilling is the drilling contractor for this well and Formation Cementing is performing the cement job. There is 1550 of 8.25 hole and 1525 of 7 surface casing in the hole. Formation Cementing began pumping at 8:46 pm and achieved good circulation of the well bore at 9:10 pm. The wiper plug was sent downhole at 9:34 pm and 11 bbl of cement returns were seen at the surface starting at 9:50 pm. The wiper plug latched down at 9:54 pm and Formations pump truck then pressured against the plug to 900 psi and held for 5 minutes. The well bled down to 880 psi over the 5 minute period, indicating a successful pressure test. Left location at 10:30 pm.
0696329370 2015-10-01 Drill / Deepen / Reopen No Violations Upon inspection, it was observed that this well has been fraced, has tubing and rods, and is awaiting a pumping unit to be set. I spoke with James Williams in person and he stated that a pumping unit will be set on this well at the same time as P#29750. The area downslope of the location has been stabilized with a vegetative cover, the area around the well head is still barren, but is compacted well and is not eroding. Mr. Williams stated they will have the entire preliminary restoration completed after the pumping unit and tank battery is set. I was accompanied on this inspection by Inspector Brian Sefcik.
1412640526 2014-10-06 Drill / Deepen / Reopen No Violations On location to retrieve cement ticket from conductor job and to check status of drilling activities. Cement around conductor could be seen approximately 2 down the annulus. Monroe Drillings rig was drilling upon arrival. The hole depth was 150 and the rig was drilling with an 8.25 tri-cone bit. The surface job for this well is anticipated to be on Tuesday 10/7/14.
