0165363688 2017-09-20 Preliminary Restoration No Violations The inspection this day revealed that this well is equipped with 8.625 inch surface casing, 4.5 inch production casing, 1.5 inch tubbing, rods, and a pumping unit. The valves on the wellhead are open at the time of inspection. The tank battery is equipped with three (3) 210 bbl. tanks, two (2) vertical separators, and two (2) meters with current charts in them. All production equipment will be shared between 29752, 29739, 29740, and 29738. All drilling equipment has been removed from site, pits have been closed, and the area has vegetation growing on it. There are no issues to report and preliminary restoration has been passed.
0294240449 2019-04-17 Production Wells No Violations On this day, I found the well equipped with 4.5 inch diameter casing and 1.9 inch diameter tubing with the production valves open. It is setup to be produced by means of a pumping unit with rods and an electric motor. The production facility consists of (3) 210-barrel steel tanks, (2) separators, and (2) orifice sales meters with current charts all contained within an earthen dike. The facility is shared with permit numbers 9739, 9740, and 9752. The tanks are properly identified and the production records are current.
0404864069 2016-06-08 Preliminary Restoration No Violations The purpose of my inspection today is a status check on a preliminary restoration. During my inspection the well is equipped with 8.625 inch surface casing, 4.5 inch production casing, 1.5 inch tubbing, rods, and a pumping unit. The 1.5 inch tubing valve is opened with gauge reading 30 psi. The 4.5 inch production casing valve is opened with the gauge reading 120 psi. The production equipment consists of three 210 bbl. tanks, 2 vertical separators, and 2 chart meters with chart dates reading 6/7/16. All production equipment will be shared between 29752, 29739, 29740, and 29738. Three frac tanks are located beside the well and one frac tank is located by the tank batteries. The preliminary restoration cant be passed until the frac tanks are removed.
1306257388 2014-10-27 Drill / Deepen / Reopen No Violations On location at 11:45 pm to witness cementing of surface casing string. There is 327 of 7 casing and 360 of 8.875 hole. Formation Cementing arrived on location at 12:00 am on 10/28/14. Circulation was seen at 1:25 am and good cement returns were seen at the surface at 1:36 am. The wiper plug was landed at 1:38 am, but the casing could not be pressure tested due to a leaky cement manifold. Pressure test is scheduled for 10:00 am. Left location at 2:00 am.
1508810087 2014-10-22 Drill / Deepen / Reopen No Violations On location at 11:45 am to witness cementing of conductor pipe. G-L Drilling was running 65 of 9.625 conductor pipe into a 75 hole with a 15 bore size. Formation Cementing pump truck arrived on location at 2:15 pm, and the bulk truck did not arrive until 3:20 pm. Pumping commmenced at 3:28 pm and circulation was achieved at 3:36 pm, but took twice as much water as it should have to circulate. The G-L Drilling Company Man stated that they encountered a soft spot approximately 25 downhole and the bit free-fell for 10 or so. The void was likely an abandoned river channel sand deposit. This was surmised due to the proximity of the location to the Muskingum River. After pumping 150 sks of class A cement into the hole, cement was never seen at the surface. Formation Cementing sent another bulk truck down to attempt to grout the hole in from the surface, but after a visual inspection, it was observed that cement was approximately 8 down the annulus of the conductor pipe. Dry portland cement was dumped in the annulus to fill the annulus to surface. Left location at 7:30 pm.
1593455491 2014-10-28 Drill / Deepen / Reopen No Violations On location at 9:45 am to witness surface casing pressure test. The casing was pressured up using G-L Drillings pressure testing setup which consisted of a well head, tee, gauge, pressure hose fitting with valve, and a 2 ball valve to relieve pressure. 5 attempts were made to pressure test the well, but all failed due to leaks in the testing equipment. Static water level was then observed within the 7 casing to determine whether or not there was a hole in the casing. The fluid level never dropped, so a different pressure testing setup was made. The new setup consisted of a 7x2 swedge, a tee, a new 600 psi gauge w/valve, and .75 pressure hose fitting w/valve. This setup allowed the entire pressure test to be isolated to the tee, eliminating leaks. The casing was pressured to 350 psi at 12:30 pm and held for 30 minutes. The casing pressure test passed with this setup. Left location at 1:15 pm.

2129029039 2019-04-10 Production Wells No Violations I was on location today to do a field inspection on this well. The well appears to be in production at this time. The valves are open from the wellhead to the production tank. There is a pumping unit on the well with rods and tubing in it. the identification for the well is located on the production tank. There are no violations at this time.
