0039795586 2016-06-29 Production Wells No Violations Upon inspection, it was observed that this well has been serviced and is now in production. Wooden conductor has been removed from the well and replaced with steel casing. I could not verify the diameter of the steel casing due to cement and soil covering it. The steel casing has been cemented in place and the annulus of the production casing is now sealed with cement. There is still some residual oil present on the soil around the well which needs to be addressed. There is a 1.25 flow line attached to the 1 gas escape line which is broken and venting gas approximately 80 from the well. No ID information is posted at the well. It is unclear which tank is being used for this well. I will contact Triad-Hunter LLC and instruct them to clean up oily soil, post ID information at the well and tank, and repair the broken flow line.
0044846587 2016-11-03 Production Wells No Violations ODNR Division of Oil and Gas supervisors Dan Goins and Eric Conrad, and I met with Triad Hunter representatives Austin Clark, Mike Horan, and Bob Handschumaker. We discussed the violation issues with this well and a timeline in which these violations shall be put back within compliance. Triad stated that they put this well back in production. A follow up inspection will need to be completed.

Triad-Hunter is in the process of evaluating this well for potential to produce or if it needs plugged and abandoned.
0735819423 2016-03-02 Production Wells Violations Noted *NOTE: THIS WELL PLOTS 1050 NW OF P#60411, AND PLOTS 100 AWAY FROM P#20037, INDICATING THE WRONG PERMIT NUMBER IS ASSIGNED TO THE WELL.* Upon inspection, it was observed that this well is capable of production, but is currently idle. The well consists of a pumping unit standing over 5.1875 casing, 2.375 production tubing with rods, and 1 tubing on a gas escape packer. The well is leaking oil on the 5.1875 casing. There is oily soil surrounding the well head and oil is visible 1 downhole on the annulus of the 5.1875 casing, where the only other casing present is wooden conductor, leaving groundwater vulnerable to contamination. There is a 100 bbl production tank with Triad IDs nearby, which appears to serve several wells on the lease. Judging by the deteriorating surface equipment, it is assumed that the well has not operated in a significant amount of time. There is no motor on the pumping unit. There is no flow line attached to the 1 gas escape tubing. RBDMS records indicate that the well has not produced oil or gas since 2012. The well shall be plugged and abandoned. Triad Hunter LLC shall submit a written plugging schedule within 30 days to Inspector Lynn Parkins at 5880 Memory Rd. Zanesville, OH 43701 and plug the well within 180 days. Triad Hunter LLC will be notified if the plugging schedule is approved or denied.
1064909436 2009-05-05 Field Inspected, Well Not Found No Violations Looked for well this day with Supervisor Stonerock. Well not found, no sign of debris. Status should be changed to PA as OGS records indicate. Triad is turning production in for this well.
1466785725 2016-06-24 Production Wells No Violations Received phone call from Bob Handschumacher of Triad-Hunter at 11:26 am. Mr. Handschumacher stated that pollution and contamination issues at this well have been addressed and that the well has been serviced and is now in production. I will perform a field inspection on the well next week to verify that issues have been corrected.
1469552982 2016-07-21 Production Wells No Violations Upon inspection, it was observed that all contaminated soil has been removed from around the well head and the well is no longer leaking oil. There is now legible and correct identification information posted at the well and production tank. The well is now in production and is being pumped on a regular basis. Mr. Bob Handshumacher of Triad-Hunter stated the leaking gas flow line is being removed this week and a pressure relief device will be installed in its place since there is currently no market for the gas. This satisfies the requirements of the compliance notice issued 3/7/2016.
