Mr. Valentine states that there is cave-in in the open hole section of this well at around 1400 which was induced by getting a sand pump lodged in the well bore. The sand pump was retrieved from the well bore. Mr. Valentine has contracted Mr. John Thompson with Jett Well Services to drill out the cave in and re-establish TD to 1675 in the Big Injun Sandstone. Work is scheduled to be perfored on the well over the next few weeks. |

1038074035 | 2015-12-15 | Production Wells | No Violations | Received voicemail message on 12/9/2015 from Paul Valentine stating this well is now producing. Upon inspection, it was observed that the well now has tubing and rods in the hole and the pumping unit is now operational and has a gasoline motor. The well was pumping at the time of inspection and there is a 6 section of 2 steel flow line coming from the production tubing which is allowing the well to pump produced water directly onto the ground. I performed a field test on the produced water using Hach Quantab low and high range titration strips. The water tested 58 ppm chlorides, which is considered fresh water. There is also a significant amount of oily/paraffin mess around the well head which needs to be cleaned up immediately. I contacted Mr. Paul Valentine and told him that all produced fluids from an oil/gas well bore must be contained. Mr. Valentine verbally agreed to cease pumping the well until a tank is set to store produced fluids. I will re-inspect this location on 12/21/15 with BLM Inspector Jim Wood and USFS Inspector Steve Sapio. |
1223829722 | 2015-03-24 | Production Wells | Violations Noted | Mr. Jim Wood of BLM and I inspected this well in response to a request from the BLM field office. Upon inspection, it was observed that this well is incapable of production and is idle. The lease road for the well is currently inaccessible to vehicles due to fallen trees blocking it. The well consists of a pumping unit standing over 6.625 surface casing. The pumping unit is pulled to the side and the rods and tubing are pulled from the well and are on racks beside the well. No flow lines are connected to the well casing. There is a 50 bbl production tank located near the well that is no longer plumbed in, but still has 3 of fluid in it. No meter was located for the well. Identification information for the well is faded and incomplete. No production records were filed for 2013. The well shall be plugged. The operator will have 180 days upon receipt of this notice to attain a plugging permit from ODNR Division of Oil & Gas Resources Management and properly plug and abandon the well. |
1450807444 | 2015-12-21 | Production Wells | No Violations | Met with BLM Inspector Jim Wood and USFS Inspector Steve Sapio to perform a follow-up inspection of this well. Upon arrival, it was observed that Mr. Paul Valentine had pulled his service rig off of the well. Mr. Valentine was working on the location upon inspection, and the well was once again being pumped onto the ground. I instructed Mr. Valentine to shut the well down immediately and he did. The 30 bbl production tank has been pumped down and a new tank location has been selected. I told Mr. Valentine he absolutely cannot pump the well again until it is plumbed into the tank, which prompted him to immediately begin running a 1.25 black plastic flow line from the well to the tank. Oily soil is still present around the well head, I instructed Mr. Valentine to, at a minimum, excavate all contaminated soil and stockpile/cover it in an area that is away from the creek until weather conditions improve to the point where a truck can be brought in to dispose of the soil. Mr. Valentine agreed to clean the location up, re-set/plumb-in the tank, and have the lease road re-graded to minimize erosion as soon as possible. |

The well has been pumped recently and the 2.375 tubing is plumbed into a 1.5 plastic flow line which leads to the 80 bbl production tank. The production tank has been set inside an earthen dike. The earthen dike has nearly 1 of rainwater in it and will need a drain and stand pipe installed to allow for periodic drainage. The location has been seeded and mulched and grass is beginning to grow in areas. No ID information has been posted. The lease road has deep ruts and washouts in areas and will need re-graded. I have spoken with Paul Valentine about the lease road and ID and he stated he will have the issues addressed when weather conditions allow for bringing equipment in. Photos taken. |
