1178919308 2013-08-27 Orphan No Violations Spent time in the Washington County courthouse, reviewing deed and lease title. Spent rest of day working on packet to submit to I/O program.
1377541038 2013-08-26 Orphan No Violations My inspection this day due to very heavy rains along Archers Fork that put flood waters up over this well, causing the release of oil. This well does not belong to Larrey Martin but is an orphan well, drilled by the Glen Clift family and last produced by Jack Eddys father Virgil, now deceased, using it to provide house gas. Jack has indicated he will do LOG as a means to get well plugged. The well is located 15 off the edge of Archers Fork Rd. and the tank sits only 10 off the road. The well is equipped with an old scissor pumping unit run by a central power unit and there is a small square tank used to hold fluids. The well has a wellhead on the 6 5/8 OD casing with 2 3/8 tubing, 1 rods. It also has 15-20 wood conductor still in place. The 6 casing likely stops at the top of the Cow Run Sand but there are no well records to indicate casing, formations or year well drilled (landowner thinks between 1906-1930s). Pictures taken, SEM is 203. Oil has been cleaned up with pads although still a small amount on vegetation.
1451567698 2015-12-30 Administrative Inspection No Violations This well has been taken off Larrey Martins bond and restored back to its HP status. With this change, we can proceed with the Orphan Well Program process. The well is and has been idle for many years and lies in the Archers Fork Run floodplain, regularly going under water and contaminating the area around it with oil from the well. The well never belonged to Larrey Martin.
