0732078258 2017-09-28 Plug / Plug Back No Violations On the day of my inspection I accompanied ODNR Inspector Brian Sefcik. Please see his comments for a detailed report. Patchwork Oil and Gas had a service rig, sacks of cement, a water truck, and a pump on location to plug this well. The well was produced from the Berea Sand. There is no record of the depth of the formation. Patchwork was able to get to a depth of 1,511 feet.
0917061299 2016-03-30 Administrative Inspection No Violations Met at Barlow Office with Supervisor Dan Goins and Triad-Hunter representatives Ben Smeltzer and Bob Handschumaker to discuss compliance issues at this well. Triad-Hunter is in the process of evaluating this well for potential to produce or if it needs plugged and abandoned.
1336601259 2017-09-29 Plug / Plug Back No Violations I was on location today to witness the bottom plug being set on this well. Patchwork Well Services is the service company plugging this well. Today I was able to witness the bottom plug being set. The company mixed the sacks of cement in two different tubs then pushed it down hole with a water pump. They originally pumped 20 sacks down hole then pulled the tubing up so they could tag the plug. After 4 hours the rig crew tagged the plug and the cement was not high enough in the hole. I had them run the tubing back down and add another 10 sacks of cement to the plug. They then pulled out the tubing from the well.
1382595382 2016-11-03 Production Wells No Violations ODNR Division of Oil and Gas supervisors Dan Goins and Eric Conrad, and I met with Triad Hunter representatives Austin Clark, Mike Horan, and Bob Handschumaker. We discussed the violation issues with this well and a timeline in which these violations shall be put back within compliance. Triad stated that they plan to plug this well.
1456858907 2016-03-01 Production Wells Violations Noted Upon inspection, it was observed that this well is idle and incapable of production. The well is open to the atmosphere and consists of 5.1875 casing and 4.5 production casing. The well casing has been severely bent over due to hillside creep over the years. The well leaks oil periodically and there is oily soil surrounding the immediate well area. There is fluid with oil on top approximately 3 down the 4.5 casing. The well is located less than 50 away from the Long Run stream bed, indicating the potential exists for stream contamination.RBDMS records show that Triad-Hunter claimed 61 bbls of oil production for the well in 2012. The well is not serviceable in its current condition and cannot produce oil or gas. The well shall be plugged and abandoned. Triad Hunter LLC shall submit a written plugging schedule within 30 days to Inspector Lynn Parkins at 5880 Memory Rd. Zanesville, OH 43701 and plug the well within 180 days. Triad Hunter LLC will be notified if the plugging schedule is approved or denied.
1464118825 2016-05-23 Production Wells No Violations Upon inspection, it was observed that the well is no longer leaking oil. The operator has clamped a 4.5 pup joint and collar onto the original 4.5 casing and has placed a threaded reducer into the collar and installed a plug and 0.5 valve. The operator has determined that the well is a plugging candidate and plans on applying for a permit to plug.
1507073775 2017-10-02 Plug / Plug Back No Violations The purpose of my inspection today was to witness plugging operations. The well was equipped with 6.625 inch diameter surface casing to an unknown depth and 4.5 inch diameter production casing to an unknown depth. Patchwork Oil and Gas planned to set an isolation plug and a surface plug through 2.375 inch diameter tubing. The isolation plug will be set from 810 feet to 610 feet and the surface plug from 500 feet to surface. During my inspection I witnessed Patchwork Oil and Gas batch mix 15 sacks of Class A Portland Cement weighing 15.6 pounds per gallon to make up a 200 foot plug from 810 feet to 610 feet within the 4.5 inch diameter casing. The plug was displaced through the 2.375 inch diameter tubing with 2.5 barrels of fresh water. Circulation was achieved during displacement and contained in a plastic 1550 gallon tank. The tubing was then tripped out of the hole and 560 feet of the 4.5 inch diameter casing was retrieved. A line was run in to tag the first plug. The plug was tagged at 760 feet in depth. The top of the plug should have been at 610 feet. The decision was made to run 2.375 inch tubing back in hole to a depth of 620 feet and set a plug to surface. 100 sacks of Class A cement weighing 15.6 pounds per gallon were batch mixed and pumped. Patchwork Oil and Gas ran out of cement. They plan to get more cement in the morning and finish pumping cement until it comes to surface.
1537797289 2018-09-24 Final Restoration No Violations The purpose of my inspection was a status check on a final restoration. On October 2, 2017 I witnessed Triads contractor, Patchwork Oil and Gas complete partial plugging operations. During my inspection today, vegetation was established throughout the lease road and the wellsite with no erosion issues and there were not any production equipment or debris left behind. The perineal stream was put back to its original state and the culvert used during plugging operations had been removed. The location meets the final restoration requirements written under the Ohio Revised Code 1509.072(B).
2111384556 2009-04-30 Field Inspected, Well Not Found No Violations Did nto find well at this location or one nearby. Pumper is mixed up on his numbering of the wells. Will have to ask him where #P-1 is. Tank over at Stephens/Phillips well has 5980 on it - believe that is where Triad thinks well is located.
