0035682617 2014-02-24 Orphan No Violations My inspection this day due to plugging of well. After leveling yard, placed water tub over hole and moved rig over the hole. Fishing tool needed to retrieve tubing which was broke off, stripped first couple joints until rods were able to be pulled. Pulled 5/8 rods and 2 3/8 tubing to a depth of 678, ran back in well with 1.5 tubing to bottom. Tagged bottom with tubing at 697.5, pulled one 32 joint and left tubing setting at 682.5 - ready to cement in morning. Rig and tub have been placed so that they are off the side of the road, placed an orange cone at corner of tub and strung some flagging.
0160028816 2012-03-19 Orphan No Violations My inspection this day required to complete I/O plug application. Twp. Rd. 48 (Bear Creek) runs along the Adams/Muskingum Twp. line and the Traver property lies in Muskingum Twp, , Lot 4 on the south side of the road. The well lays 2-3 off the edge of the road and 3 away from a large tree with the well situated between the two. The well has been hit by a car/road grader/snow plow and is slightly bent over. The well is about 35 past the driveway of the homeowner. The well is equipped with 1 and 2 3/8 tubing and I believe the well was used for house gas. The production casing is 5.5 OD and there may be surface casing sized at 9 5/8. Well records have not been found but the well is probably Macksburg 500 or Cow Run and less than 1000 deep. The well could very likely be shot and may have been drilled up to 100 years ago. More wells exist to the back of the property and up the hill where there are 2 old wooden tanks. Mr. Traver would like these 2 wells plugged.
0420059238 2008-04-17 Orphan No Violations Orphan well sitting 5 off side of county road - has been hit by snow plow or other vehicle, still has scissor jack on it but bent of and 1 tubing in well folded over. Neighbor says this was very good gas well at one time - currently not leaking. Landowner requests well put on I/O list as feels is a hazard. I/O packet ready to go when get permit no. 2nd well on property that landowner says he might as well keep - when open valve, much gas blows - did not die back - has been used at one time for house gas. Not sure when last produced. MIKE - need permit no.
0605488061 2014-04-16 Final Restoration No Violations My inspection this day was due to the receipt of the Landowner Waiver regarding restoration. The property owner wants the responsibility to restore the areas affected by the plugging operations from this point on and will release contractor from any further responsibilities regarding reclamation of his property. To date, the tree removed in order to plug this well is still on site, to be cut up and disposed of by the landowner, all plugging equipment and materials have been removed and the areas affected by the plugging operations have been rough graded. Reclamation with regards to the Contractor is complete - the landowner will finish any further reclamation from this point on.

1373566511 2013-07-11 Orphan No Violations I received call from Supervisor that Jeremy Huck had called, saying the well along road had got covered with water from the rains and released oil from the hole. My inspection found that the water had been very high and about 25% of Mr. Hucks grass in yard has oily film on it - odor is strong. I also found a very slight rainbow on the creek about 25 away and everything covered with a film of oil about 700 down the road on either side. Gas meter had been submerged and large piles of trees and other debris washed downstream last evening with the rain. During flooding, creek must have 70-80 wide. Jeremy has got pictures from last evening, I took pictures this morning but oily film is hard to see. Only found one spot where brown oil was laying in a 1 x 2 pool below the well. Jeremy is interested in the Landowner Grant program for his other well up by the house but this well should be considered emergency funding with Coast Guard probably paying the bill for plugging.

1380901912 2013-10-04 Administrative Inspection No Violations I received a call from Steve Wolfe from FEPA stating that they were not allowed to work due to the government shut-down, responding to my e-mail this morning to request field inspections for our Coast Guard wells, particularly this well. He said to go ahead and plug it and that we could request re-embursement of funds after the well was plugged and not to hold off plugging just because they hadnt inspected it.
1381923085 2013-10-15 Orphan No Violations I received a call from the property owner, Jeremy Huck, inquiring when plugging operations would commence. I explained to him that the Travers #1 had been approved for emergency funding and the Orphan Well Program was also trying to get the Travers #2 approved for plugging under a different contract. He was adamant that both wells be plugged concurrently or he would not let us on the property. I referred him to the Orphan Well Program Manager, Gene Chini, to discuss further.

It looks good and will take only some minor finish grading before it will be ready to be seeded. The snow cover made it difficult to see if new gravel had been applied to the driveway. The property owner stated he had already bought one truck load of gravel. Until it warms up, not much more can be done to the lawn and drive.
