0014406545 2016-05-20 Production Wells No Violations On location with Nate Lane and Trisha Cartmell to check status of oil spill cleanup. Upon inspection, it was observed that oil absorbent booms are still in place in the creek. Very little oil was observed in the stream, and was confined to designated recovery areas. Oil is still seeping from the creek bank near the former tank location. The siphon dam in the creek is still functioning properly. Photos were taken. A meeting has been set up for 10:00 am on 5/24 with Marilyn Haynes and Ronnie Earley to discuss building containment dikes and putting the lease back in compliance.
0199133370 2016-06-17 Production Wells No Violations On location with Nathan Lane to follow up on oil spill cleanup. No free oil product was observed in North Fork Mill Creek and in the majority of the small tributary affected by the spill. Oil is still slowly seeping out of the creek bank under the old leaking tank location. Oil absorbent booms, pads, and peat moss near the tank location are in need of removal and replacement. The lease road has been graded and is in better condition than that last inspection. New tanks have yet to be set for the well. New photos taken.
1193448277 2016-04-27 Production Wells No Violations On location with Geologist Jake Glascock to grab water samples for VOC and Oil and Gas analysis. Samples were taken to the ODNR Environmental Lab in Cambridge. Cleanup crews are still on location and are actively recovering free oil product from the waterways. I witnessed the removal of the compromised 100 bbl production tank from the location. A Littman Excavation rolloff box was brought onto location for transport and disposal of used oil absorbent material and contaminated soil. Emergency Operations Coordinator Nathan Lane is continuously monitoring cleanup efforts.
1252165471 2016-05-16 Production Wells No Violations Upon inspection, it was observed that free oil product is only visible around oil absorbent pads and booms in the streams. Slight sheen is visible in other areas where there are eddys in the streamflow. The siphon dam in the creek is functioning properly. Seeded and mulched areas are beginning to grow grass. The lease road is still soft and has deep ruts. The lease road will need re-graded once it dries out. The well will not be pumped until it is plumbed into a competent tank. The well is still supplying domestic gas to the Haynes residence. Photos taken. I will continue to monitor the progress of this location toward compliance.

1462379410 2016-04-26 Production Wells No Violations On location with Supervisor Dan Goins, Geologist Jake Glascock, and Emergency Operations Coordinator Nathan Lane to check on oil spill cleanup efforts at this well. Cleanup crews are working around the clock to recover free oil product from North Fork Mill Creek and its small tributary. Booms, pads, and peat-sorb are being monitored and changed out as necessary. The production tank for the well is being pumped down today and the tank will be removed from location. All production tanks on the property will be pumped empty and inspected for integrity. All damaged or leaking tanks will be replaced. Significant progress has been made toward recovering free oil product. A skeleton crew will remain on location overnight to monitor booms and pads.
1495656618 2016-05-10 Production Wells No Violations Upon inspection, it was observed that the majority of free oil product has been recovered from the streams. The only places where free product is visible are the gathering areas that were established to make recovery easier. There is still oil leaching out of the creek bank where the leaking 100 bbl tank was located. There is a siphon/underflow dam in place downstream of the old tank location. There are pads and booms in place behind the siphon dam. The old tank location has been re-graded, limed, seeded, and mulched. Pads and booms are being monitored and changed as needed. Photos were taken.
1602379206 2016-06-27 Production Wells No Violations On location with Nathan Lane to meet with Trevor Irwin of OEPA DERR in regard to oil spill cleanup efforts at this well. Upon inspection, it was observed that there is no more oil sheen present in North Fork Mill Creek and aquatic life is thriving. Mr. Irwin stated he is considering cleanup complete, but recommends keeping some oil recovery controls in place to intercept any residual oil that works its way out of the creek bank near the old tank location. Some oil is still leaching out of the creek bank. Oil/sediment saturated booms present in the small tributary stream need to be removed and disposed of. Marilyn Haynes has yet to supply new tanks to be set on the location so the well can be put back into production. Compliance will continue to be monitored. Photos taken.
1607881558 2016-08-18 Production Wells No Violations Upon inspection, it is observed that oil spill cleanup efforts are nearly complete. Trash bags full of oil soaked booms and pads are still laying near the creek bank in areas and need to be picked up and disposed of. Marilyn Haynes was notified of the booms and pads and she stated they will be cleaned up as soon as possible. The well is not yet plumbed into a production tank and it is not being pumped. The well is providing domestic gas to the Marilyn Haynes residence. Ms. Haynes states she has purchased tanks to set for the well and others on her property, however there is currently a dispute with the contractor who is hired to set them and build containment dikes. No identification information has been posted at the location as of yet. I am accompanied on this inspection by DOGRM E.R. Coordinator Nate Lane.
1670253918 2016-05-24 Production Wells No Violations On location with Nate Lane and Trisha Cartmell to meet with Marilyn Haynes and Ronnie Earley in regard to compliance issues with this well. After discussing options, it was decided that a 100 bbl production tank would be set near this well which would serve it as well as P#27221. The 210 bbl production tank which is currently in use will be pumped down and removed from location. The mechanical separator and meter run will be set near the well head at P#27221.The tank will be set a minimum of 50 away from the creek and will have a containment dike built around it to meet SPCC guidelines.
