0432197251 2009-07-09 Preliminary Restoration No Violations Site has been graded and topsoil replaced. It does not appear as though any seeding was done but this is a farm field. Stone access road remains from C.R. C about half way to well location. Preliminary restoration acceptable.
1154647512 2010-02-04 Drill / Deepen / Reopen No Violations DMRM was contacted by the Bryan Fire Department, due to uncontrolled leaking gas from well head. Well was damaged by unknown assailant who drove oversized off road 4X4 truck over well and bent 4.5 inch casing and 2 inch tubing to the ground. The valves on the well head for the 4.5 inch casing were broken completely off and leaking uncontrolled gas. The threads for the gate valve on the 2 inch tubing were stretched and leaking gas. Local FD evacuated at least three homes in the down wind direction. Roger Frantz and Dave Barndt were contacted for assistance. They removed the reaming threads from the well head replaced with new valves. Well was then allowed to be blown down. Bryan FD relinquished control of site to MRM Inspector. The well was allowed to flood with formation water. The Operator mobilized Universal well service to load well with brine. A brine truck delivered fluids and an empty tanker truck was used to displace fluids into during the well control operation. The well was to be plugged the following day by other personnel.
1240426214 2009-04-22 Production Wells No Violations Well completed 4/17/09. Drill rig down but all equipment on site. No restoration on site yet. All drainage controlled on site. There are 2 plastic lined pits on site filled with fluid. Spoke to John Suhar by phone. Rig will move to well 34-171-20065-0000 tomorrow.
1240432299 2009-04-15 Production Wells No Violations Spoke to operator Spike. Surface casing cemented by Halliburton on 4/14/09. No problems with cement job. Picked up cement ticket. BOP installed and tested to 500 psi.
1242665540 2009-05-14 Production Wells No Violations Pits backfilled on site. Some grading has been done but recent rain has stopped activity. Operator on site working on well. Acid was added to well recently.

1759145989 2010-03-31 Final Restoration No Violations Well was plugged in February 2010 due to accident when truck ran over well releasing H2S gas. Checked site for final restoration. Spoke to landowner Victoria Benner. She would like to talk to operator about stone remaining in drive and field. Contacted Don Kreager of David R Hill Inc. He will call Mrs Benner and determine what she wants done with the stone. Will obtain written consent if drive is to remain permanent. Well was cut off and ID plate attached 4 feet below ground according to Don Kreager. Field is planted in wheat. Small area of disturbance around well has been graded. Will review site again for final restoration after stone road issue resolved.
