0255671181 | 2015-09-09 | Orphan | No Violations | At the time of the inspection I was on location with Bryan Smith and Dave Albrecht of the Division of Real Estate. They were on location to survey the well and I was on location to provide directions and monitor gas levels during the survey. The well was venting Hydrogen Sulfide gas from out the top of the 20 by 8 plastic riser pipe as intended. A faint smell from the gas was evident 20 to 80 south of the well but no gas levels were detected with the Division issued gas detector around the location. Winds were out of the north at 10 mph and the temperature was 78 degrees. |
0312229138 | 2015-11-04 | Orphan | No Violations | At the time of the inspection I was on location with Terry Whitehair, Emergency Operations and Response Regional Manager for an Organizational & Safety Meeting. This meeting is to discuss access through the park and safety concerns for the plugging of the well scheduled for November 16th. Glenn Davis of Glenn Davis Well Service, Mike Przysiecki of the Bowling Green Parks and Recreation Department, Bowling Green Fire Chief Tom Sanderson, Wood County Emergency Management Agency Director Brad Gilbert, along with other representatives of city and county government and first responders were in attendance. A slight odor of Hydrogen Sulfide gas was evident 200 to the east of the well as winds were out of the west at 15 miles per hour. A hand held gas meter was used to attempt determine the levels of Hydrogen Sulfide present but due to high winds all gas has dissipated and no levels were detected. Mobilization for plugging of the well is scheduled to begin on November 16th. |

We also discussed the setup of an Organizational & Safety Meeting scheduled for November 4th. A slight odor of Hydrogen Sulfide gas was evident 100 to the west of the well as winds were out of the east at 12 miles per hour. A hand held gas meter was used to attempt determine the levels of Hydrogen Sulfide present but due to high winds all gas has dissipated and no levels were detected. |

0800485479 | 2015-08-25 | Orphan | No Violations | At the time of the inspection all risers when the well was discovered were still in place and in the same condition. There was still a small amount of gas leaking from the well and escaping from the top of the riser as designed. A faint odor of Hydrogen Sulfide was detected 50 east of the well but no gas reading were detected at the well or downwind from the well. Winds at the time of the inspection were at 12 mph from the west. |

0898713111 | 2012-05-14 | Orphan | No Violations | At the time of my inspection the Hydrogen Sulfide gas level above the riser was at 10 ppm and at 140 ppm inside the riser. An 8 plastic riser was placed inside the 12 and raised about 15 above grade to help disapate the gas. Also fencing was placed around the well and will be left in place and monitored until plugging arrangements can be made. |

1122587582 | 2015-09-25 | Orphan | No Violations | At the time of the inspection the well was still venting Hydrogen Sulfide gas from out the top of the 20 by 8 plastic riser pipe as intended. A faint smell from the gas was evident 40 to 100 south of the well but no gas levels were detected with the Division issued gas detector around the location. Winds were out of the east at 10 mph and the temperature was 80 degrees. |
1337859073 | 2012-05-04 | Orphan | No Violations | At the time of my inspection I was on site in response to Complaint # 3762. Troy Sonner, City of Bowling Green Engineer, called regarding a dead spot of grass in Simpson Garden Park. The location in question is 15 south of a split rail fence that marks the north boundry of south section of the park. Gas levels of 18 LEL were detected and a metal detector indicated some steel at the location. |

1399993777 | 2014-03-25 | Orphan | No Violations | At the time of the inspection there was a 10 diameter PVC riser placed over the well from its discovery back in 2012. This riser extends to 15 above grade to allow for the venting of the Hydrogen Sulfide gas leaking from the well. There is a 16 x 16 split rail fence around the well that was put in place by the maintenance crew of the park. The winds on this day were approximately 10 mph from the west and a faint odor could be detected approximately 60 to the east. |

1486736568 | 2017-02-10 | Final Restoration | No Violations | At the time of the inspection the Final Restoration had been completed and the area was being used by Simpson Garden Park. The area around the plugged well had established grass and seven new trees were planted around the location. A new cement walkway had been completed by the park in the fall of 2016 where the access lane to the well had been. Final Restoration Passed. |

1932275154 | 2012-05-11 | Orphan | No Violations | Well associated with Complaint # 3762. The well is located about 400 east of Wintergarden Rd. and 15 south of split rail fence. Site was excavated by the City of Bowling Green with Troy Sonner and others on site. The well has 8 casing filled with dirt about two feet below ground level with about 1 yard of impacted soil. No wellhead or other production equipment on site. Hydrogen Sulfide gas level above well at 34 ppm. A 12 plastic riser was placed over well and area was filled back in. Old records indicate well might have been plugged in 1976. Also well # 20347 is located about 400 to the west and was previously plugged with fire clay in the late 70s and replugged with cement in 2007 to a depth of 650. I reviewed well for the Orphan Well Program. |
