0278090508 2015-10-01 Orphan No Violations At the time of the inspection the plugging contractor, Second Oil Ltd., had continued drilling at 8:00 am at a depth of 291. Drilling continued until 5:00 pm at a depth of 502. There was no gas detected at any point during todays drilling.
0406899835 2015-10-08 Orphan No Violations At the time of the inspection all plugging operations were on hold until the plugging contractor, Second Oil Ltd., returns to work on 10-12-2015. The well was shut in on 10-7-2015. During the inspection there was no pressure on the pressure gauge and no visible or audible leaking from the wellhead.
0473566305 2015-09-30 Orphan No Violations At the time of the inspection the plugging contractor, Second Oil Ltd., started and completed mobilization of equipment to the well by 10:30 am. Well setup was completed at 11:15 am by rigging up the 3.5 bit, stem, and jars inside a 22 section of 4.5 pipe used as a wellhead extension. This was done because of wood and other debris encountered two feet into the well. Drilling started with the cable tool rig at 11:15 am and was stopped at 1:15 pm to remove the 4.5 wellhead extension. Drilling with the same tools continued at 1:30 pm and ended at 5:00 pm at a depth of 291 feet.
0932849162 2015-09-18 Orphan No Violations At the time of the inspection the plugging contractor has cut the 6 and 8 casings at 1 foot above ground level. There is solid cement in the annular space between the two casings and fresh water in the 6 casing about 8 down. No gas was detected using the state issued hand held gas meter. A 7 collar was welded to a flange that was welded to the 8 casing. A 7 to 2 swage was put in the 7 collar. A 2 valve was added and to the top of the swage and the well was shut in.

1443104058 2015-09-23 Orphan No Violations At the time of the inspection the well was shut in with a 2 valve from 9-18-2015 and no leaking from the welded fittings was detected. The well was then opened by the plugging contractor, Second Oil Company, to determine if any pressure was building up in well. There appeared to be no pressure when the well was opened at the 2 valve but gas levels of 1.75 Methane and a LEL of 22 were detected with the state issued gas meter. The 2 valve was closed and the well will remain shut in until the well is plugged.
1443798137 2015-10-02 Orphan No Violations At the time of the inspection the plugging contractor, Second Oil Ltd., had continued drilling at 8:00 am at a depth of 502. At 10:30 am, wood chunks and pulp were bailed from the well then drilling continued. More wood was bailed from the well throughout the day. At 4:00 pm, drilling stopped at a depth of approximately 515 and the well was shut in. There was black water covering the line and tools when bringing the tools back out of the hole but the tools were clean of debris. At 4:30 pm, the Plugging Crew and Inspector left location. There was no gas detected at any point during todays drilling.

1444843498 2015-10-14 Final Restoration No Violations At the time of the inspection the Plugging Contractor, Tony Kryder & Tommy Kryder, Second Oil Ltd., arrived on location. A plate was welded on the top of the well with the API # 61314 welded on top. All equipment was moved off location to Maumee Stone #1. Frantz Enterprises Ltd. was on location for three hours to move brine to the next location and take ~55 barrels of return water and oil for disposal. All Restoration of the location will be completed by Maumee Stone as detailed in the Orphan Well Plugging Contract.
