At the time of the inspection the crew started drilling with the 4 cable tool bit at 9 am. The fresh water flow was diverted to the existing drain tile. The water flow could be moved to the half tank in the event anything other than the existing fresh water in encountered. An obstruction was hit at 60 and again at 151. These obstructions were broken through and a total depth of 1462 was reached. The crew then ran 1430 of 1.5 tubing with a 5 mud anchor in preparation for plugging on 11-5-14. A crew from OBriens Safety Services was on location to monitor gas levels. There were readings as high as 55 ppm Hydrogen Sulfide at the wellhead during the cleanout. Levels around the well and work area stayed between zero and 15 ppm of Hydrogen Sulfide. |

The flow continued to slow as the brush and stone plug set on 11-10-14 at about 60 was added to over the next 4 hours. The final length of this plug was from 60 to 45 and 35 of 1.5 tubing was set above that. A heavy mix of 50/50 Pozmix with 4% calcium added was pumped down the well until returns were observed at the surface. A total of 60 sacks was used and the cement was weighed by the inspector at 15.1 lb./gal. The level inside the well settled at about 6 below grade with no sign of gas or water. A crew from OBriens Safety Services was on location to monitor gas levels. There were readings as high as 4 ppm Hydrogen Sulfide at the wellhead when the well was opened that morning. Levels around the well and work area stayed between zero and 2 ppm of Hydrogen Sulfide throughout the plugging. |

This water tested at a 2 on a low range quantab and flowed into a drainage tile in the middle of a 10 x 12 depression. The path of this drainage tile is unknown but slow flowing puddles of water were evident to the north stretching an undetermined distance. The temperature at the time was below freezing but these puddle had not frozen over. A low range quantab reading of 4 was obtained from one of the puddles approximately 50 north of the well. A follow-up inspection will be needed to determine the extent of the flowing water due to heavy snow still on the ground. |

1375974318 | 2013-07-31 | Orphan | No Violations | An inspection of this orphan well revealed a strong and consistent flow of artesian water and the release of H2S from the inside of an approximate 6.5 inside diameter casing (ID). The flow rate was not measured (Refer to video clip for details) but amount of water released was discharged just as quickly into a second casing. The immediate area surrounding the well consisted of a depression in the ground surface (i.e. resembled a pit approx 5x 5) with pooled water and a layer of white film. A second casing was also observed within this depression approximately 5 away and contained a yellow filter style device for debris on the top that is often seen for drain tile. The fluid that was being released from the approx 6.5 (ID) casing drained into this casing. A measurement of this casing could not be obtained due to the fluid level and presence of H2S. The immediate area also contained a concrete monument likely from a standard rig that measured 10 x 5 x 2.5. The well was monitored with a gas meter equipped with an automatic pump, a probe and filter. A strong rotten egg odor was encountered and the H2S reading from this well reached a maximum of 36 ppm. The well was situated on property designated as agricultural field with no crops planted at this time, no residential dwellings within approximately 500, and located off a state route. A GPS reading was obtained and recorded for the potential entry off state route. Central Office will be notified of H2S status for this county and township. |
