2729380 Primary Facility 2018-05-14 Incident- Response to Accident or Event Violation(s) Noted Inspection in response to an incident reported by the Operator on Friday May 11, 2018 at 6:57 pm. The Operator reported a 75 gallon release of fluid to the ground. The Operator indicated that DEF (diesel exhaust fluid) was lost when a fork lift operator lost control of a tote while loading the material onto a truck. The incident resulted in approximately 75 gallons of DEF to be released to the gravel well pad near the GPU control panel located on the left side of the well site. At the time of inspection the impacted area was excavated and placed in a waste roll off box awaiting proper disposal. The Safety data sheet was on site. The PPC plan was on site and the ER plan was on site. The Department recommends that the Operator continue to follow the Departments current spill policy regarding the remediation of spills greater than 42 gallons.
