Well appeared to have been drilled, but was not equipped for production. Excavated drill pit located onsite and it contained a small amount of fluids. There was no observed accelerated erosion or sedimentation in relation to the access road or gas well. Well pad had revegetated and the vegetative cover met the 70% uniform coverage criteria. At this time the Department is requesting that the operator forward a completed Well Record and Completion Report for this well. This documentation should be forwarded to Water Quality Specialist Greg Higgins no later than May 17, 2012.

2765016 Primary Facility 2018-08-14 Plugging(Includes Plugged/Mined Through) No Violations Noted This well had been spudded to 20' deep and had a piece of 6 5/8" conductor pipe sanded in with a 6"x2" swage and a valve installed on top. The operator lost the spacing to drill this well due to another operator drilling a well first on the neighboring property too close to drill this well. The operator plugged this well by cutting the 6 5/8" casing off 2' below ground level, filling the 18' piece of casing in with cement to the top of the pipe and the operator then placed a metal monument plate on top of the pipe with the well API number on it and buried the plate. The operator also owns the property which this plugged well is located on. On the date of this inspection report I inspected the site of this plugged well and found it to be properly reclaimed and growing vegetation over the plugged well.
